Culvert repair materials have been supplied to the Government by Contech Engineered Solutions and shall be provided to the Contractor (see list of Government-provided materials in the attached Drawings and Exhibits). All other necessary materials (aggregate base, structural backfill, concrete, etc.) shall be provided by the Contractor. The Dark Canyon Culvert was damaged in May 2025, most likely due to the insufficient compaction of structural backfill during initial installation. The Contractor shall repair the existing 26'-0" span, 16'-7" rise single radius arch multi-plate culvert per the guidance of the manufacturer of both the original structure and the Government-provided culvert repair materials, Contech Engineered Solutions. This includes two (2) concrete collars at the joints of the original and repair materials. After the culvert has been repaired and road subgrade has been reconstructed to match the legacy design grades and elevations in the attached Drawings and Exhibits, an aggregate base surface course will be placed and compacted. Lastly, for stabilization, hydraulic seeding and mulch will be placed on fill slopes and other areas of disturbance. All work shall be in accordance with the contract requirements. Contractor shall, without additional expense to the government, be responsible for obtaining any necessary licenses and/or permits which comply with any federal, state, municipal laws, codes, and regulations applicable to the performance of the work. Equipment will be considered clean when visual inspection does not reveal soil, seeds, plant material, and other such debris. Disassembly of equipment components or specialized inspection equipment is not required. Contractor shall notify the Contracting Officer at least five days in advance of moving equipment in so that arrangements can be made for inspections. Contractor is responsible for temporary traffic control and construction signs shall be in accordance with the latest version of MUTCD. 2.0 Background Dark Canyon Multi-plate pipe arch culvert was built back in 1999/2000. The pipe arch experienced a catastrophic failure in May of 2025 when a multiplate seam failed and caused a large sinkhole to appear the full width of the roadway. This was determined to likely be caused by insufficient compaction of structural backfill during installation. The forest, while consulting with Contech Engineered Solutions, removed the backfill and removed the damaged pipe arch materials and came up with a fix consisting of adjoining the end sections of old pipe with the new middle section of pipe with concrete collars at the joints tying the pipe arch together as one unit. Working with Contech the Forest has the necessary multi-plate steel and components needed to rebuild the pipe arch, which will be Government provided property under this contract. 3.0 Location The project is located within the Hayfork R
