Solicitation No. 75H71526Q00015 Biomedical Equipment Inspection COMBINED SYNOPSIS/SOLICITATION: THIS SOLICITATION IS A 100% SET-ASIDE FOR SMALL BUSINESSES. This is a combined synopsis/solicitation for commercial products/services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Indian Health Service (IHS), Nashville Area Office intends to award a firm-fixed price commercial product/service Requirements Contract in response to this Request for Quotation (RFQ) for Biomedical Equipment Inspection Services for the Indian Health Service, Mashpee Wampanoag Health Service Unit, Mashpee, MA. The solicitation documents & incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation Overhaul. The associated North American Industry Classification System code is 811210 and the small business size standard is $34.0M. PRICE SCHEDULE: Biomedical Equipment Inspection: the quoted pricing must be all inclusive (to include but not be limited to labor, materials, equipment, tools, travel, lodging, per diem, fringe benefits, federal, state, and local taxes) plus all other costs pertinent to the performance of this contract. Please complete the attached price schedule utilizing your most competitive and reasonable rate: PURPOSE OF THE CONTRACT: Biomedical Equipment Inspection. The vendor shall provide services in accordance with the attached Performance Work Statement. PERFORMANCE WORK STATEMENT: See Attached PERIOD OF PERFORMANCE: The estimated period of performance of this contract will be as follows: Base Year: August 1, 2026 – July 31, 2027 Option Year One: August 1, 2027 – July 31, 2028 Option Year Two: August 1, 2028 – July 31, 2029 Option Year Three: August 1, 2029 – July 31, 2030 Option Year Four: August 1, 2030 – July 31, 2031 INVOICE SUBMISSION AND PAYMENT: The Contractor shall comply with HHSAR Clause #352.232-71 “Electronic submission of payment requests” located in the Provisions and Clauses section of this solicitation. SYSTEM FOR AWARD MANAGEMENT (SAM) REGISTRATION: All quoters must have a UEI (Unique Entity ID) number, be registered and active in the System for Award Management at https://www.sam.gov, and have completed their SAM Representations and Certifications online in order to be eligible for award of the contemplated contract. Please include your active UEI number with your quote. QUOTE SUBMISSION INFORMATION: To be considered for award, vendors must quote on all products/services listed in the solicitation. Quote submissions must include the following items: Sections 17a, 23, 24, 30a, 30b, and 30c completed per the attached SF 1449 document (include UEI number). Attached Price Schedule completed. Mashpee Service Unit is in need of a contract to fulfill the annual inspection requirements for the medical equipment utilized on site. Objective The objective of this biomedical equipment inspection is to verify that all medical devices are functioning safely, accurately, and in compliance with applicable performance standards and manufacturer specifications.
