STATEMENT OF WORK
Blood Bank Testing Reagents
I. Introduction
Hershel “Woody” Williams VAMC Department of Pathology and Laboratory Medicine Services has a need to acquire blood bank testing reagents. These reagents are needed to perform transfusion testing that permit patients to receive blood and blood products safely, effectively and in a timely manner.
The medical center is seeking to establish a new base plus four option year contract with the period of performance as follows:
Base Year: 10-18-2026 to 10-17-2027
Option Year One: 10-18-2027 to 10-17-2028
Option Year Two: 10-18-2028 to 10-17-2029
Option Year Three: 10-18-2029 to 10-17-2030
Option Year Four: 10-18-2030 to 10-17-2031
II. Contractor Responsibilities
Provide reagents necessary to conduct transfusion testing.
Manifest/invoice of reagents shipped.
Ship reagents as per the schedule.
(d) A reagent user guide or similar documentation will be provided. The manual shall include the testing methodology used for each reagent, reagent reference ranges, and specimen requirements and any special handling required.
(f) Transport container in such a manner as to ensure the integrity of the reagents Contractor will supply any special preservatives required for reagent preservation. All reagents shall be shipped in containers provided by contractor.
(g) Contractor shall provide the VA laboratories with a means of communication to permit immediate inquiry regarding the status of pending reagent delivieries.
(h) The VA Laboratories reserves the right to request the results of any proficiency testing that the contractor subscribes.
(i) Contractor shall provide monthly billing for tests provided by the 20th of each month following the month in which the service was delivered.
III. Quality Control
Contractor shall notify the VA Laboratory Director (or designee) immediately by phone or email of any quality errors or delivery disruption. The notification shall be followed up by a written statement with recommended actions to take to remedy or resolve the situation. If the Contractor has defects or errors in their reagents that caused erroneous results, the Contractor shall supply replacement reagents at no charge to the VA or provide a credit to the VA. The choice of which will be discussed between the VA and the Contractor.
For quality purposes, tests routinely performed in duplicate should be indicated.
IV. Testing Changes
The Contractor shall advise the Hershel “Woody” Williams VAMC Laboratory Director (or designee) of any changes in reagent methodology, procedures, reference values.
