Description

STATEMENT OF WORK Project Description and Purpose: Contractor shall provide all labor, materials, equipment, and management to Replace, ceiling lighting 1st, 3rd, and 6th floors of FLC general areas FLC W-143 at Naval Station Norfolk. The Contractor shall ensure proper operational status upon completion of task. Notes: N/A Attachments: N/A Contractor shall: 1. Submit a detailed schedule of work. 2. Submit outages request that are applicable to the task. 3. Submit a fall protection plan for all work over six feet. 4. Attain signed Hot Work Permits if applicable to the task. Any cutting, welding, soldering or grinding will require a hot work request approved by the Base Fire Department and posted at the work site. 5. Ensure that surrounding surfaces are protected from damage throughout the duration of the task. 6. Perform necessary Lock Out/Tag Out of all effected equipment to ensure all power sources are properly de- energized. 7. Keep a nice neat working space, making sure all flooring and surrounding areas are free and clear of tools and material. 8. Convert all lighting to LED lighting. 9. Replace all LED emergency lights where needed. 10. Remove all ceiling fans in hall ways. 11. Remove Can lighting in ceiling tile in area Cap off electrical wiring to ensure safe proper status where needed. 12. Remove all Speakers in Ceiling tile and cap off electrical wiring. 13. Perform a worksite cleanup and properly dispose of all debris associated with the task. Working Hours: The Contractor shall provide a detailed schedule to accomplish all work. All work must be performed during normal working hours. If afterhours work is needed, it must be approved by the Federal Government. Pre-Proposal Meeting: A pre-proposal site meeting shall be held if requested by the Contractor prior to the award of this task order. The contractor shall coordinate the date and time with the Contracting Officer. Pre-Performance Meeting: No work will start prior to the Pre-performance meeting with the Government Designated Authority and customer. The purpose of the meeting is for the contractor and government to obtain a mutual understanding of the existing conditions, the tasks required to complete the work, specific impacts and risks associated with the work, and the level of effort required developing the work plan, and covering all documentation requirements. The Contractor shall start work immediately after the pre-performance meeting. Hazardous Material Disclosure: For work in soil excavation and other exterior construction etc., the Contractor may be required to work with contaminated material involving abatement or management of these contaminated materials. However, if during the performance of the work, the Contractor encounters hazardous substances not known prior to issuance of this Task Order; the Contractor shall immediately stop work and inform the Contracting Officer for direction. In no instance shall the Contractor perform the work involving the hazardous material without notifying and receiving approval from the Contracting Officer for direction. Document and Repair Schedule: The Contractor shall start work immediately after pre-performance meeting. The Contractor shall provide to the Government Designated Authority (GDA) a list of all personnel who will be working on site, Activity Hazard Analysis (AHA), Accident Prevention Plan (APP), schedule of work and any outage paperwork at the Pre-performance meeting. All work shall be performed in accordance with current standard industry practices, applicable local, state and federal codes, laws and regulations, Unified Facilities Criteria (UFC’s), Unified Facilities Guide Specifications (UFGS), US Army Corps of Engineers (EM 385), all OSHA safety -- 1 of 2 -- regulations, EPA guidelines and base procedures. All work shall comply with the Uniform Federal Accessibility Standards (UFAS) and Americans with Disabilities Act Accessibility Guide (ADAAG). All work including subcontracted work shall comply with base procedures, latest revision of the NNSY specific guide specifications for environmental and safety requirements, state and federal regulations. -- 2 of 2 --
