Elevators Preventive Maintenance Gallup Indian Medical Center, Gallup, NM 87301 Statement of Work Services include: The Contractor shall provide Preventive Maintenance for a total of 6 (Six) Elevators. Four Elevators at Gallup’s Main Hospital Building (Building # 2000) and Two Elevators at the Outpatient Clinic Building (Building # 2018). The Preventive Maintenance in this Customized Professional Maintenance Program Service IV includes inspection and service of the following for any operating problems: Building 2000: Gallup Main Hospital Building (4 Dolver Elevators Passenger Traction) Serial Number 9409062344 9409071647 9409072062 9409072349 Building 2018: Outpatient Building (2 TKE Hydraulic 3-Stop Unit Elevators) Serial Number ER4227 ER4228 Contractor shall provide Monthly inspections of the 6 (six) Elevators to ensure continuous operations Contractor shall complete Preventive Maintenance work per current ASME A17.1 Safety Code for Elevators and Escalators, Section 8.6 requirements Monthly inspection needs to include the following but not limited to: Customer Notification Initial Site Inspection Check, lubricate, and adjust elevators on a systemic and regular basis Repair or replace all above ground parts subject to wear as necessary Clean the car top and hoist way pits Conduct safety tests as outlined by National Building Code Complete Preventive Maintenance on Exhaust Fans Keep completed and detailed records of elevator system and all work performed Contractor shall provide emergency service 24 hours a day, 365 days a year. Additional 35 service callbacks per year are included in the proposal per year. All callbacks and/or emergency calls that exceed the number per year as stipulated above shall be charged as overtime service requests and are billable at the attached billing rates; This line item includes trip charges/travel costs, lodging costs and OVERTIME pay per pre-established hourly rates 120 hours (5 business days) prior notice must be given to the POC contact listed below before conducting any services (to coordinate for security badges, parking and staging areas for equipment if needed) Provide a complete hard copy and electronic Service Report for completed Preventive Maintenance. Copy of service Report must be left onsite with an appropriate signature. Contractors are required to be on-site during Annual Fire Alarm Testing of elevator shaft detectors at no additional cost. Alarm testing contractors will need access to elevator shaft. Train and provide recommendations to Gallup Maintenance Staff on the best solutions to continuously maintain Elevators in excellent conditions Contractor shall develop and maintain a comprehensive safety program. All Contractors’ personnel onsite must use Personal Protective Equipment (PPE) that meets OSHA requirements Contractor shall comply with the Hospital policy including Pre-Construction Risk Assessments (PCRA), which includes Interim Life Safety Measure (ILSM), Infection Control Risk Assessments (ICRA) and Above Ceiling Permits while performing work on IHS property. Any scratches or other damage caused during construction of this project must be fixed immediately by contractor. All work areas shall be maintained in a clean and orderly condition with NO piling of demolished materials. In accordance with IHS Circular 2016-04, Mandatory Seasonal Influenza for Construction Contractor Personnel, the contractor shall insure that all its employees and subcontractors possess proof of influenza vaccination for any workers that will be on the IHS facility from October 1st through March 31st at no additional cost to the government. Proof of vaccinations shall be provided to the COR or designee. All contractors’ personnel will be accompanied by IHS employee(s) from Maintenance, Security, or Environmental Services (Housekeeping) or other departments while onsite to comply with security regulations (Background Checks), Chaperone Form. Contractor Qualification Requirements: Contractor shall be licensed to perform work in the state of New Mexico. Contractor shall have a DUNS number and registered within SAM.gov. INCLUDE LINE ITEM FOR PAYING TAXES ON LABOR ONLY ALL Contractor personnel MUST check-in and check-out with any of the Points of Contact above before and after conducting any work on hospital premises Fee Schedule: Contractor shall indicate below the Total Cost to perform stated services. Total cost shall be inclusive of labor, travel, per diem, tools, equipment, technical knowledge, materials, supplies and federal/state/local taxes and all necessary expenses to perform Preventative Maintenance (PM) and unscheduled maintenance. Contracting Officer Representative (COR): The COR is responsible for: Monitoring the Contractor’s technical progress, including surveillance and assessment of performance and recommending changes in requirement to the Contracting Officer. Performing technical evaluation as required. Performing technical inspections and acceptance required by this contract; and Assisting the Contractor in the resolution of problems encountered in performance.
