Statement of Work (SOW) FPSU Fire Alarm System Testing, Inspection A. GENERAL INFORMATION 1. Title of Project: FPSU Fire Sprinkler and Alarm System Testing and Inspection 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the tasks and deliverables described in this Statement of Work (SOW). The contractor shall complete all onsite testing and inspection as required by NFPA 72, Chapter 14, Inspection Testing and Maintenance of the 2010 Edition. This includes but is not limited to completing all quarterly and annual testing and inspection requirements. 3. Background: The Fort Peck IHS Service Unit clinic has three separate clinics. Below are locations, estimated square footage and make/model of the fire panels. (Note Verne E Gibbs and White Buffalo share a main panel) • Chief Redstone Clinic 18000 SF Make: Pyrotronics Model: MXL IOX • Verne E. Gibbs Clinic 24000 SF Make: Edwards Model: EST3X • White Buffalo Clinic 12000 SF Make: Edwards Model: EST3X Testing of the fire alarm system is required by the accreditation agency The Joint Commission and must be completed according to NFPA 72, 2010 Edition. B. CONTRACTOR REQUIREMENTS 1. Testing and Inspection Contractor shall be responsible for completing all testing and inspection listed in NFPA 72, Table 14.3.1, 2010 edition with a required frequency of quarterly and annually. All items listed with a frequency of annually shall instead be completed quarterly during each quarterly inspection. Contractor shall test 100% of the devices shown in the list below: Chief Redstone Clinic  Main Fire Alarm Panel  5 Pull Stations  37 Smoke Detectors  1 Heat Detectors  4 Duct Detectors  4 Combination Fire & Smoke Dampers  1 Remote Annunciators  13 Horn Strobes  17 Strobes -- 1 of 3 -- 2 Verne E Gibbs Clinic  Main Fire Alarm Panel  16 Pull Stations  22 Smoke Detectors  2 Heat Detectors  2 Remote Annunciators  22 Horn Strobes  24 Strobes White Buffalo Clinic  Main Fire Alarm Panel  4 Pull Stations  1 Smoke Detectors  2 Remote Annunciators  21 Horn Strobes  17 Strobes 2. Scheduling and Access Site visits shall be made quarterly and should be a schedule and completed on one of the dates listed for each quarter. Contractor shall schedule site visits a minimum of 7 days in advance with the Facility Engineer or Maintenance Supervisor. All testing work that will require activation of the horns and strobes shall be completed between the hours of 6am and 8 am. During each site visit all deficiencies that need addressed should be noted and relayed to the facility engineer so appropriate action can be taken. C. GOVERNMENT RESPONSIBILITIES The contractor will have access to the facility as needed to complete service work. An effort shall be made to not interrupt the flow of patient and staff during clinic hours. The facility is closed on all federal holidays and the contractor will not be granted access on these days. Facility Engineer or Maintenance Supervisor will be available to provide access to secured areas. Facility Engineer or Maintenance supervisor will NOT be responsible for showing or helping find the individual device locations for the Contractor each time they are onsite. Contractor will have access to onsite as-built drawings and O & M manuals. -- 2 of 3 -- 3 D. DELIVERY OR DELIVERABLES Contractor shall provide a minimum of 4 site visits including travel and labor costs for each site visit. 1. Reporting Requirements a. The contractor shall provide the owner with a detailed written report showing each device tested and list the value measured or pass/fail status. Report should be completed in accordance with NFPA 72, 2010 Edition. b. The report shall be transmitted electronically in PDF format and a paper copy shall be mailed to the facility engineer at the address shown below. E. TRAVEL Work is to be performed at the following locations: Chief Redstone Clinic 550 6 th Ave North Wolf Point, MT 59201 Verne E Gibbs Clinic 107 H Street Poplar, MT 59255 White Buffalo Clinic 805 Assiniboine Ave Poplar, MT 59255 F. CONTRACTOR EXPERIENCE REQUIREMENTS – KEY PERSONNEL Contractor shall be in the fire detection system installation and maintenance business and have a minimum of 5 years’ experience. Technician sent to site to complete Testing and Inspection shall meet requirements of NFPA 72 14.2.2.5, 2010 Edition. Proof that technician meets the NFPA requirements shall be provided to the Facilities Engineer prior to any onsite work taking place. -- 3 of 3 --
