Statement of Work General Construction related Supplies and Materials for the KCVA “In-House Construction” Projects A. Background: The KCVA Campus is undergoing a reorganization and modification of its space in the current facility by moving clinical spaces around the facility. These projects open up space in the main hospital for other clinical use. This reduces congestion in clinic space, allowing for better patient care for the patients who will use the new clinic space and for our patients who come to the hospital for care. B. Description: The Blanket Purchase Agreement (BPA) is to streamline and standardize the purchases of construction related materials during multiple phases of medium-to-large renovations and new construction projects performed by the KCVA Construction Crew over a five-year period. The procurement of these materials will involve coordination with the Construction Supervisor of the KCVA. After the materials are ordered, the KCVA will require the supply contractor to coordinate with the KCVA Construction Supervisor who is the Contracting Officer Representative (COR) and/or a construction team Site Superintendent as designated by the COR. Coordination discussions will address material delivery, verification of product purchase and receipt, and tracking of supplied materials C. Scope of Requirements: Provide KCVA with all general construction, HVAC, plumbing, electrical supplies, and materials in support of various medium to large renovations and new projects. More details can be seen in Attachment B. Product delivery, purchase, delivery verification, and tracking support shall be discussed and coordinated with the COR and/or designated Site Superintendent. These projects vary in size from smaller scale to larger multi-phased renovations, new construction projects and Move Committee projects. These are for multiple areas within the KCVA Medical Center and/or surrounding Center Based Out-patient Clinics (CBOC). Projects listed below are subject to change due to priority needs of the Medical Center and surrounding CBOC’s. • 589-24-999 – Miscellaneous Construction Projects • 589-24-998– Move Committee Rooms Along with the procurement of these materials, the contractor will be expected to properly deliver, track material purchases and deliveries, and verify delivery. This includes:  Be involved in construction meetings when asked.  Provide visual graphs and reports of the material tracking.  Material purchase verification  Material delivery verification  End user support/tracking.  Complete material documentation Material and Supply Procurement The materials required and specific quantities for each project will be identified at the time of order. All items must be brand-name or equal, and if an equal item is proposed, the contractor shall provide the corresponding product specifications for review. After the materials are purchased, the contractor and KCVA will coordinate within three (3) days to review and finalize all orders and determine the required delivery locations. The contractor shall also coordinate with the KCVA Construction Team to ensure they are informed of all delivery times, methods, and any other requirements necessary for providing the materials. -- 1 of 4 -- Once the material order is finalized and the product has been placed, the contractor will work with the KCVA Construction Team to schedule delivery based on availability and project timelines. As part of supply purchasing and tracking, the vendor must provide the construction team with a standard spreadsheet and project schedule within 10 days of delivery to support visualization of current and upcoming phases, as well as material tracking and purchasing trends A summary of what should be provided during the current and next phase is below:  Microsoft Excel™ for material tracking. Delivery Delivery of the material shall be arranged according to the phasing and schedule of the renovated or new construction projects. Supply and material may not be delivered and stored at the KCVA earlier than 2 weeks before the scheduled delivery date. The contractor shall provide all equipment, labor and material necessary to deliver all the ordered supply and material identified in this SOW. All supply and material shall be delivered to a designated area at the medical center. The KCVA medical center is located at: KCVA Medical Center, 4801 E Linwood Blvd, Kansas City, MO 64128 D. Material/Supply Definitions: D.1 Construction: The product descriptions and estimated quantities are provided in Attachment B. Specific products/materials and exact quantities will be identified at the time of ordering, up to the maximum amounts listed in Attachment B. I. Wall Insulation Cavity wall insulation is used to reduce heat loss through a cavity wall by filling the air space with material that inhibits heat transfer. This immobilizes the air within the cavity (air is still the actual insulator), preventing convection, and can substantially reduce space heating costs. II. Acoustical Ceiling A ceiling covered with or built of material with special acoustical properties. III. LVT (Luxury Vinyl Tile), which offers 3D printing technology which adds depth and realism, durability, with a high-performance wear layer to a vinyl tile product. IV. COVE BASE Cove base tiles are usually used at the base of tiled walls in lieu of tile baseboards, to create a seamless and watertight transition between the wall and the floor. V. Handrail/Crash rail Thick protectors that extend horizontally along a wall to prevent carts, light machinery and personnel from causing damage during daily operations and maintenance. VI. Terrazzo Base Terrazzo is a composite material, poured in place or precast, which is used for floor and wall treatments. VII.
