B.2 PERFORMANCE WORK STATEMENT
The Contractor shall provide comprehensive full-service maintenance, routine and emergency repair service for the government owned sterilizers, washers, dryers and other related equipment located at the Salt Lake City VA Medical Center.
Contract service shall ensure that the equipment functions in conformance with the original manufacturer’s specifications and the latest published edition of NFPA-99, UL, OSHA and VA manuals.
All equipment shall be operable and available for use 95% of the normal operating hours of the equipment, which are 8:00 AM-5:00 PM, Monday through Friday, excluding national holidays. Downtime will be computed from notification of the problem during normal work hours. Scheduled maintenance will be excluded from downtime. Operational uptime will be computed on a monthly basis. Repeated failure to meet this requirement may subject the contractor to default actions.
All service/repairs shall be performed during normal working hours unless requested or approved by the COR or designee. Work performed outside of normal working hours at the request of the COR or designee will be billed at the contractors prevailing hourly service rate.
Unscheduled Maintenance (Emergency Repairs)
Contractor will provide emergency repair service between regularly scheduled services and calibration. Priority technical support and remote monitoring will be provided 24 hours a day, 7 days a week for the duration of the contract.
The Field Service Engineer (FSE) must respond with a return phone call to the COR or designee within 30 minutes of receipt of telephone notification of an issue, 24 hours a day. If the problem cannot be resolved over the phone, the FSE will arrive on-site and commence repairs no more than 4 hours after receipt of notification that the problem remains unresolved.
The contractor will only use parts that are equal to or exceed OEM specs.
Scheduled Maintenance
The contractor shall perform scheduled PM services semi-annually on all equipment unless otherwise noted, ensuring that all equipment performs in accordance with the original manufacturer’s specifications and the latest published edition of NFPA-99, UL, OSHA and VA manuals. An outline of the PM procedures and schedule will be provided to the COR within two weeks of award of contract. Contractor will utilize manufacturer recommended procedures and checklists, documenting work performed and actual values obtained (as applicable) during each PM and will provide this documentation to the COR at completion. PM services shall include, but need not be limited to, the following:
1) Cleaning of equipment
2) Reviewing operating system software diagnostics to ensure that the system is operating in accordance with manufactures specifications
3) Calibrating and lubricating the equipment
4) Performing remedial maintenance of non-emergent nature
5) Testing and replacing faulty and worn parts and/or parts likely to become faulty, fail or become worn.
6) Inspecting electrical wiring and cables for wear, replacing as needed.
7) Inspecting all mechanical components such as door assemblies, gaskets, steam and water valves, solenoid valves, pressure regulator valves, steam, water strainer, jacket and chamber strainer, air filter cartridge, chamber and jacket traps, printer assembly, battery-backed RAM, buzzer, water level sensor, cooling fan, fan filter, air connection, steam, water and drain connections, door lock piston, bearings and tracks, interlocks, motors, keypad, air pressure, door latch switches, distilled water reservoir, EO gas vent line and moisture trap. All listed items shall be replaced as needed.
8) Annual chamber cleaning of all steam sterilizers
9) Returning equipment to operating condition in full compliance with specs at completion of PM.
10) Providing documentation of services performed.
11) PM services shall be provided in accordance with the PM schedule provided herein. Any exceptions must be arranged and approved in advance with the COR or designee.
12) All parts, services, manuals, tools or software required to successfully complete the PM schedule are included within this contract unless specifically stated in writing otherwise.
Parts
The contractor shall furnish and replace parts as needed to meet uptime requirements. The contractor will have ready access to all parts, including unique and/or high mortality replacement parts. All parts shall be compatible with existing equipment and meet or exceed all manufacturer specs. Rebuilt, used, or parts removed from another piece of equipment shall not be installed. This contract will include all parts required to maintain required uptimes unless specifically excluded in writing.
Service Manuals/Tools/Equipment
The contractor will supply all tools, test equipment, service manuals and diagnostic software to meet all performance standards of this contract.
Documentation/Reports
Documentation shall include detailed description of all parts and services provided for scheduled and unscheduled (i.e. emergency repairs) services performed. Each report must document the following data in detail:
1) Name of the contractor and contract number.
2) Name of Field Service Engineer performing the service
3) Date, times (start and finish), equipment downtime, and hours on site for service call.
4) VA Purchase Order number if service is provided outside of normal working hours.
5) Detailed description of reported problem.
6) Identification of equipment serviced: Inventory number, Mfr. Name, device name, model number, serial number or any other manufacturer identification numbers.
7) Itemized description of service performed, including any costs associated with parts and/or services performed outside of normal working hours.
Reporting Requirements
