STATEMENT OF WORK (SOW) Project Title: Lagun CM-4 Series #60211 Repair and EvaluationPurchase Request (P/R) Number: 2126406Y2266A26001 1.0 SCOPE The Contractor shall provide all labor, supervision, materials, parts, tools, equipment, and travel necessary to perform repair services and technical evaluation on one (1) Lagun CM-4 Series Milling Machine (Serial Number #60211). The work includes, but is not limited to, the repair of the X-Axis, a comprehensive evaluation of the Y-Axis Ram Extension, and a control upgrade, in strict accordance with (IAW) the technical parameters outlined in Republic-Lagun Quotation #1952. 2.0 PERIOD OF PERFORMANCE (POP) The Period of Performance shall be established and finalized upon contract award. The Contractor shall commence work upon receipt of the formal Notice to Proceed (NTP) or as otherwise directed in writing by the Contracting Officer (KO). 3.0 PLACE OF PERFORMANCE All on-site repair and evaluation services shall be performed at the following location:United States Coast Guard (USCG) Yard2401 Hawkins Point RdBaltimore, MD 21226 4.0 SPECIFIC REQUIREMENTS AND TASKS 4.1 Equipment Repair: The Contractor shall execute repair services utilizing Original Equipment Manufacturer (OEM) or fully compatible supplied parts for the Lagun CM-4 Series X-Axis.4.2 Material List: The Contractor shall furnish and install all required parts as detailed in Republic-Lagun Quotation #1952, including but not limited to Schneeberger track rollers and guiderail blockers.4.3 System Evaluation: The Contractor shall conduct a diagnostic evaluation of the Y-Axis Ram Extension. Should additional parts or repairs be identified during this evaluation, the Contractor shall notify the Contracting Officer’s Representative (COR) before proceeding with any out-of-scope work.4.4 Control Upgrade: The Contractor shall execute the control upgrade as delineated in the approved service plan. 5.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES (GFP/GFE/GFS) 5.1 Material Handling: The Government shall provide material handling services to load/unload Contractor machinery and assist with equipment setup. The Contractor shall submit requests for material handling services to the COR a minimum of forty-eight (48) hours in advance.5.2 Utilities: The Government shall provide reasonable access to water, compressed air, and electricity. Compressed Air: The Government shall provide a compressed air source. The Contractor shall be responsible for providing compatible hoses and hookup connections. Electricity: The Government shall provide electrical services up to 200 amps and up to 480 volts.5.3 Conservation: The Contractor shall instruct its personnel in utility conservation practices and ensure Government utilities are not wasted. 6.0 CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES 6.1 General: The Contractor shall furnish all equipment, parts, tools, and services required to execute this SOW that are not explicitly designated as Government Furnished in Section 5.0.6.2 Site Attendance: Contractor personnel shall report to the designated work site on the date(s) specified in the written notification provided by the Contracting Officer. 7.0 INSPECTION AND ACCEPTANCE 7.1 Post-Maintenance Inspection: The Government (via the COR or designated Quality Assurance Evaluator) will inspect and review the completed work in its entirety to ensure compliance with this SOW prior to the approval of any invoices. All work is subject to final inspection and acceptance by the Government.7.2 FAR 52.237-1 Site Visit: Offerors are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance. Failure to inspect the site shall not constitute grounds for a claim after contract award. 8.0 SECURITY AND ACCESS REQUIREMENTS 8.1 Installation Access: All Contractor personnel must utilize the RAPIDGate Program or possess a valid Transportation Worker Identification Credential (TWIC) if repeat access to the CG Yard is required for seven (7) or more days annually.8.2 Credentialing Costs: All costs associated with obtaining RAPIDGate/TWIC access are the sole responsibility of the Contractor.8.3 Access Roster: The Contractor shall provide a list of all employee names and Dates of Birth (DOBs) scheduled to perform work on-site along with their quotation. (For access questions, contact 1-877-727-4342).8.4 JCP Certification: Joint Certification Program (JCP) certification is mandatory for United States or Canadian contractors requiring access to unclassified technical data. 9.0 SAFETY AND ENVIRONMENTAL COMPLIANCE 9.1 Safety Standards: The Contractor shall strictly adhere to all applicable OSHA Shipyard Industry Standards (29 CFR 1915), ISO 9001, and ISO 14001 standards.9.2 Personal Protective Equipment (PPE): The Contractor shall provide all necessary PPE for its employees.9.3 Emergency Services: In the event of an emergency, Contractor personnel shall dial 410-636-3993 from non-Yard mobile phones or extension 3000 from internal Yard landlines.9.4 Environmental Protection: The Contractor shall comply with all federal, state, and local environmental regulations, as well as CG Yard environmental plans.
