Overview and Objective
Minot AFB is home to two major Air Force units: the 5th Bomb Wing and the 91st Missile Wing. Minot AFB is located approximately 13 miles north of Minot, North Dakota and approximately 120 miles north of the North Dakota state capital, Bismark. Minot is known for pleasant, warm to moderately hot summers and bitterly cold winters. At times, the area can have extreme winds, with wind speeds exceeding forty miles per hour occurring regularly. 5th Communications Squadron needs periodic maintenance performed on SSMD-2mm disintegrator. The Squadron has supplies on hand to be used for the periodic maintenance.

Scope of Work
Vendor must complete periodic maintenance inspection and repair for any need issues identified with the equipment. The Vendor must contact the Government’s Point of Contact at least 5 days prior arrival to verify availability to access the equipment. Vendor must dispose of any old parts or waste materials.
Requirement
Vendor must complete required periodic maintenance action on SSMD-2mm disintegrator per recommended technical specifications.
Warranty
The Contractor must supply the standard commercial warranty.
Special Requirements
6.1 Base access requirements: Contractor Access to Department of the Air Force Installations The contractor shall obtain base identification, and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Department of the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid REAL ID, current vehicle registration, valid vehicle insurance certificate to obtain a vehicle pass. ▪ Contractor personnel who do not possess a REAL ID must present a valid U.S. passport with the State’s ID. Personnel who do not possess a valid U.S. passport must present: (1) original or certified copy of their birth certificate, (2) their original social security card, and (3) marriage certificate or divorce decree as applicable.
7 Routine
7.1 The Lead Technician will be on site within 72hrs starting from approval by the COR. Within 6 hrs from arrival, the technician will complete troubleshooting and make the needed repairs, NTE $100 for parts per call. If the system cannot be returned to operational status within 6hrs and under the $100 parts call threshold included in the Service Call, the Contractor shall provide the COR with a plan to repair before the Contractor leaves the site. The estimate and schedule for the proposed Service Order are required within 48 hours of the Service Call unless otherwise negotiated by the CO.
8. Service Order
Once the Technician arrives on-site and determines the extent of the repair required, the Contractor shall notify the CO/COR of the scope and cost of the repairs. If the repairs cannot be made within the time 6 hours or cost NTE $100 for parts per the PWS the Contractor shall submit a SO for CO and COR evaluation and approval. The official repair SO document shall include the relevant facts and proposed costs for the situation.
