1 Performance Work Statement Propane Service Peach Springs Health Center, Peach Springs, AZ 86434 1.0 Performance Objectives The Contractor shall ensure the continuous and reliable supply of propane service to all designated locations for the entire period of performance, utilizing their best commercial practices to meet or exceed the performance standard listed in the Quality Assurance Surveillance Plan (QASP). • Period of Performance: 7/18/2026 – 7/17/2031. • Locations: Service is required for the following accounts: Location Service Address Account Number Main Clinic 943 Hualapai Way 46074237 Government Quarters 1 940 Hualapai Way 46074186 Government Quarters 2 938 Hualapai Way 46074186 2.0 Performance Standard The critical performance standard for this objective is maintaining an uninterrupted supply of propane. • Standard: Zero instances of service interruption or stock-outs (running empty) at any of the specified facilities during the period of performance due to contractor action or inaction. 3.0 Contractor Flexibility The Contractor is responsible for all logistics, inventory management, delivery scheduling, and routing necessary to consistently meet the "zero interruptions" standard. The government will not dictate delivery schedules or methods, only the required outcome. 4.0 Government Monitoring The Government (specifically the PSHC Facility Manager or COR) will monitor the Contractor's adherence to the performance standards through periodic account verification and energy audits as required by the QASP. -- 1 of 2 -- 2 Quality Assurance Surveillance Plan (QASP) Sample This QASP outlines how the Peach Springs Health Center I.H.S. will evaluate the Contractor's performance for ensuring a continuous propane supply to all designated locations, as defined in the Performance Work Statement (PWS). 1.0 Performance Objective and Standard Performance Objective PWS Paragraph Performance Standard Acceptable Quality Level (AQL) Ensure continuous and reliable propane supply to all locations 1.0, 2.0 Zero instances of service interruption or stock-outs 100% compliance (zero deviations accepted) 2.0 Surveillance Methods The Contracting Officer Representative (COR) or PSHC Facility Manager will perform the following surveillance to ensure compliance with the performance standard: • Periodic Monitoring: The COR/Facility Manager will conduct monthly visual checks of tank levels at the Main Clinic, Government Quarters 1, and Government Quarters 2. • Customer Feedback: The COR/Facility Manager will record any complaints from facility tenants or staff regarding loss of propane service. • Account Verification/Audits: Periodic reviews of delivery slips and energy audits will confirm service continuity and billing accuracy. 3.0 Documentation and Reporting Any failure to meet the AQL (i.e., a service interruption or stock-out) will be documented in a Performance Assessment Report (PAR). This report will be provided to the Contractor for corrective action. 4.0 Incentives/Deductions Performance below the AQL may result in contract deductions or further actions as outlined in the contract terms and conditions -- 2 of 2 --
