1 Performance Work Statement (PWS) Luke Medical Group (MDG) and Human Performance Team (HPT) 12 MAY 2026 1.0. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, and all labor necessary to provide laundry & scrub rental and cleaning service, including pick-up and delivery at Luke Air Force Base, AZ for the items listed in and Appendix B, “Packaging and Method of Finish and Appendix C “Estimated Workload Data.” Performance shall be in accordance with the scope of this Performance Work Statement (PWS) and other specifications or provisions of the contract. Pickup and delivery points are listed in Appendix D, “Pick-up and Delivery Schedule.” 1.1. SCOPE OF WORK. Non-personal services for laundry and dry-cleaning services for Luke AFB, Arizona. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to perform Laundry and Dry-Cleaning Service as defined in this Performance Work Statement (PWS). The Contractor shall perform to the standards outlined in this PWS. 1.2. SPECIAL REQUIREMENTS. The Contractor shall meet all Joint Commission (JC) and Healthcare Laundry Accreditation Council (HLAC) rules and regulations covering the handling of clean and soiled healthcare linens and be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in the healthcare laundry/linen cleaning service. 1.3. SERVICES SUMMARY. SPECIFIC TASKS. 2.1. PICKUP AND DELIVERY. The Contractor will pick up and deliver linens/scrubs as designated in Appendix D, “Pick Up and Delivery Schedule.” 2.1.1. PICK UP/DELIVERY DAYS AND HOURS. The Contractor shall pick up and deliver all linens/scrubs Tuesdays and Thursdays (MDG) and Tuesdays (HPT) between the hours of 0730 AM -1030 AM, excluding Federal holidays. If the holiday falls on a pickup/delivery day, the Contractor shall pick up linen the next business day after the holiday (i.e. holiday falls on Monday, pick up of linen will be on the next business day, which is Tuesday). The Contractor shall pick up linens/scrubs at the locations designated in Appendix D, “Pick Up and Delivery Schedule” and Appendix E, Federal Holidays/Down Days”. 2.2. WORKMANSHIP AND SANITATION. Contractor delivered finished linens/scrubs shall conform to the generally accepted industry standards of quality for cleanliness, finish and appearance. Laundered/dry cleaned articles are to be clean, dry, and free of lint, odor, spots, stains and wrinkles. -- 1 of 9 -- 2 2.3. RE-PERFORMANCE OF LAUNDRY/DRYCLEANING. Notwithstanding the provision of this contract covering inspection, acceptance and deduction from payment; any item found to have been cleaned unsatisfactorily shall be re-cleaned and delivered at the next scheduled pick up/delivery date. 2.4. PACKAGING. All wrapping shall be accomplished as designated in Appendix B, Packaging and Method of Finishes”. 2.5. SCRUBS REPLENISHMENT. Semi-annually, the Contractor and Luke MDG will conduct a scrub reset. The Luke MDG will update the staff member inventory list to see if any changes in sizes or personnel have occurred. At that time, an inventory of the scrubs will be conducted. Once both inventories have been completed, a complete reset of scrubs will occur. The Contractor will pull/reduce the inventory and weekly cost where needed or increase the inventory and weekly cost where needed. If any scrubs are found to be missing, the Luke MDG will reimburse the Contractor at a replacement cost. 2.6. HPT LINEN REPLENISHMENT. The Contractor and HPT will inspect the linens every 2 months to ensure linens are usable and in good condition. If any towels exhibit the following, they will be removed from circulation: permanent stains, discoloration, loss of absorbency, or any condition deemed unsanitary for guest and patient use. Additionally, on an annual basis, the Contractor and HPT will conduct a linen reset. At that time, an inventory of linens will be conducted. Once both inventories are completed, a complete reset of linens will occur. 2.7. HOURS OF OPERATION: Normal Hours of Operation. The Contractor shall perform the services required under this contract during the following hours, excluding Federal Holidays and Down Days: Monday – Friday, 0730 AM – 1030AM. See Appendix D, “Pickup and Delivery Schedule” and Appendix E, “Federal Holidays/Down Days”. 3.0. GENERAL INFORMATION. 3.1. SECURITY REQUIREMENTS. The Contractor shall comply with security and base access requirements as outlined in Attachment “Contractor Access to Air Force Installations.”. Contractor employees will be required to obtain and display identification badges. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice. 3.2. QUALITY CONTROL. 3.2.1.
