The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary except as specified in paragraph 3.0 as Government Furnished, to perform PHA services, as defined in this PWS. 1.1 Scope The main objective of this contract is to conduct and complete annual medical requirements for the New York Army National Guard (NYARNG) Service members (SMs) in accordance with Army Regulations 40-501, 502,40-3, Department of the Army Pamphlet (DA PAM) 40-502, DoDI 1322.24, and 6490.03. The contractor shall perform annual Periodic Health Assessments (PHAs) with accompanying Mental Health Assessment (MHA) for the NYARNG in support of routine annual and mobilization-specific requirements. 1.2 Background For more than a decade, the National Guard Bureau (NGB) and NYARNG have used contractors to complete PHAs and MHAs through the Defense Health Agency (DHA) Reserve Health Readiness Program (RHRP). Effective October 2026, these services will no longer be funded at the national level and will be pushed to various Title 10 Medical Treatment Facilities (MTFs) to reduce overall cost associated with required medical activities. This contract serves as a bridge to ensure no lapse in contracted services over the next three years. The overarching goal is to ensure continuity of operations in lieu of the RHRP contract which previously provided these services. The RHRP contract met requirements which could not be organically staffed by NYARNG military providers. The contractor shall develop, administer, train, and coordinate a statewide integrated network of private sector Health Care Professionals (HCPs) who can provide this capability at the request of the NYARNG. These providers may be Medical Doctors (MD), Doctor of Osteopathic Medicine (DO), Physician Assistants (PA), and Nurse Practitioners (NP). The contractor is encouraged to (and it is highly recommended) use prior service or currently serving military HCPs to streamline training for new contracted providers and maximize the efficiency of each event. The contractor will provide these services in-person ONLY at specific locations throughout the State of New York. Virtual or telehealth is NOT authorized under this contract. 1.3 Period of Performance. This contract is a 3-year IDIQ contract with Task Orders issued on an as-needed basis. Payment will be made on a per-capita basis and not on an event-based payment schedule. 1.4 General Information. 1.4.1 Quality Control (QC). The contractor will develop and maintain an effective Quality Control Plan (QCP) to ensure services are performed in accordance with this PWS. The QCP is how the contractor assures its work complies with the requirements of the contract. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The QCP shall include a detailed discussion of the steps proposed to ensure quality, adherence to the schedule, and proactive communication. At a minimum, the contractor shall develop QC procedures which address the areas identified in Technical Exhibit 2 within the Performance Requirements Summary (PRS). The QCP shall include a description of the corrective action steps to take should the contractor’s performance be substandard. An organizational depiction of the project team and chain(s)-of-command shall also be included in the QCP. The contractor shall provide a preliminary QCP with their proposal. The proposal shall include a single point of contact who may resolve quality concerns. This individual will have the express authority to act on behalf of the contractor regarding all quality issues and/or concerns including human resources and personnel management issues. The finalized QCP will be accepted by the Government within 10 days of the contract award. After acceptance of the QCP, the contractor shall obtain the Contracting Officer’s (KO’s) acceptance in writing of any proposed changes to its QCP. 1.4.2 Quality Assurance (QA). The government will evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan shall be primarily focused on what the Government must do to ensure the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and acceptable quality level(s) defect rate(s). 1.4.3 Performance of Services. The contractor shall provide pre-event coordination of requirements with government personnel between the hours of 0900-1700 EST Monday through Friday, except on recognized U.S. holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closing, or similar government-directed facility/installation closings.
