Description

1 REAGENT RENTAL AGREEMENT FOR THE PHOENIX INDIAN MEDICAL CENTER (PIMC), INDIAN HEALTH SERVICE The Health and Human Services (HHS), Indian Health Service (IHS), Phoenix Indian Medical Center (PIMC), located in Phoenix Arizona, is seeking proposals from qualified vendors to provide a diagnostic testing instrument for measurement of arterial, venous and pleural pH, PC02, P02, S02, tHb, F 02Hb, FCOHb, and FMetHb. The instrument must include Data Management software. The vendor shall provide a Lease Agreement that includes an instrument that performs fully automated, on-demand arterial blood gas analyses, including all Quality Controls, Reagents and Supplies as well as a warranty to cover the lease term. The instrument must include Data Management software with all upgrades and license renewals for the term of the lease. The contract shall be for one (1) base year plus four (4) option years. The Contractor shall deliver a Blood Gas Analyzer to the Phoenix Indian Medical Center, 4212 N. 16th Street, Phoenix, Arizona, 85016, within 30 days of the award date The annual volume of tests for Blood Gases is approximately 800, Calibrations occur every 30 minutes and automatic Quality Control level every eight (8) hours. The services of the Blood Gas Lab are available 24 hours/day, 7 days/week. This testing service will be provided to all departments and outpatients clinics of the PIMC. Scope of Work The Contractor shall provide a five (5) year Lease Agreement commencing from the date of the award of the contract. The Contractor shall provide a Blood Gas Analyzer instrument that performs fully automated, on-demand Blood Gas testing for measurement of arterial, venous and pleural pH, P02, PC02, tHb, F02Hb, FCOHb, and F MetHb, with the following features: • Fully automated sampling • Fully automated sample mixing • Automatic Quality Controls • Automatic Calibrations • Non cartridge-based system The Contractor shall provide Data Management software with all upgrades and license renewals for the term of the lease. The Contractor shall provide installation of analyzer and training for PIMC Respiratory Therapy staff at no additional cost. The Contractor shall provide a warranty with instrument service that must include and cover labor, travel, parts and software upgrades at no cost to the Government. The Contractor shall provide Quality Controls, Reagents and Supplies for 800 tests per year that is required to perform testing on the instrument. -- 1 of 2 -- 2 The Contractor must deliver reagents and supplies quarterly. The Contractor shall provide, when needed, expedited delivery of supplies identified. The Contractor must provide notification to the IHS for items placed on backorder for more than 7 days from the date ordering. If there are any changes in product availability and production and/or specifications of the instrument, the Contractor shall provide a notice in writing to the designated Contracting Officer Representative (COR). The Contractor must be able to fulfill urgent delivery requirement for overnight delivery. The Contractor must allow return or provide a full credit for items with expiration dates, which allow an unreasonable shelf life.
