DEPARTMENT OF VETERANS AFFAIRS – Eastern Colorado Healthcare Boiler Plant Domestic Hot Water Heater Bundle Repair STATEMENT OF WORK 1. Background The Rocky Mountain Regional VA Medical Center (RMRVAMC) has three (3) Aquaplex domestic hot water heaters that provide efficient and reliable hot water heating for various applications throughout the facility. The purpose of this service contract is to have a contractor furnish all labor, materials, supplies, tools, equipment, and qualified personnel to replace Aquaplex tube bundles or equivalent tube bundles along with control valves sized for current load in the Boiler Plant while adding three secondary steam isolations. Equipment is located in the Boiler Plant at the Rocky Mountain Regional VA Medical Center, 1700 N Wheeling Street, Aurora, CO 80045. 2. Objective: Replacement of one (1) set of tube bundles Replacement of three (3) sets of control valves on Aquaplex domestic hot water heaters Add three (3) secondary steam isolation to Aquaplex domestic hot water heater 3. Scope: Proper disposal of all spent tube bundles and control valves Check plumbing for proper isolation of each domestic water heater Installation of the new tube bundles, properly and according to specifications Installation of the new control valves, properly and according to specifications Verify and check all operating parameters are within safe levels Provide start-up and training of the system to VA employees Test and ensure full operation is restored Complete detailed report for review and training of the units 4. Contractor Qualifications and Responsibilities: Contractors shall be qualified and skilled employees with previous work experience in removal, rebuilding and replacing Aquaplex tube bundles or equivalent tube bundles. Contractors shall be qualified and skilled employees with previous work experience in removal, rebuilding and replacing control valves and secondary steam isolation Contractors must demonstrate having performed labor, start-up and commissioning of these units The contractor shall furnish all spare and replacement parts needed to service the equipment. The price of all parts used to perform all scheduled preventive, and unscheduled maintenance shall be included in the coverage. The Government will not pay for any parts provided (during either scheduled or unscheduled maintenance), by the contractor. Only new standard parts (manufactured by the maker of the equipment, or equal) shall be furnished to maintain the equipment. The contractor shall provide tools, (test) equipment, service manuals or service diagnostic software. The contractor shall obtain, have on file, and make available to its FSE's all operational and technical documentation (such as operational and service manuals, schematics, and parts list) necessary to meet the performance requirements and repair of the medical equipment. A reference list of applicable. Contractor shall take all precautions necessary to protect people and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees’ fault or negligence. Contractors shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. The Government reserves the right to accept or reject Contractor’s employees for the rendering of services. Complaints concerning Contract Personnel’s performance or conduct will be dealt with by the Contractor and POC with the final decision being made by the Contracting Officer. 6. Report Requirements: Furnish report within one week of each facility inspection in “Microsoft Word” format by email to the FMS POC. 7. Resources: The Government will not furnish any resources for the fulfillment of the services required under this contract. The contractor must provide all tools, equipment, materials, transportation, professional expertise, personnel, and supervision for maintenance services required under this contract. The contractor shall remove all debris and trash from facility upon completion of work. 8. Hours of Work and Scheduling Scheduling The work will be performed Monday through Friday from 7:30am to 4:00pm. The majority of the work will have minimal impact on the operation of the VA Medical Center. Any potential disruption to ongoing medical center operations shall be coordinated 2 weeks in advance and shall be subject to review and approval by the CO/POC. Any other disruption of operations in any building shall be scheduled at least one week in advance with the CO/POC. Work shall be scheduled to allow continuous operation of all facilities to the greatest extent possible. Contractor Check-In All Contractor personnel working onsite at the VA facility shall check in and out with the POC located in Building L (ENC). Access to work areas, and performance monitoring, will be provided by the POC. In all cases, the CO/POC shall be notified of any changes which may affect the response time of vital equipment. 9.
