Statement of Work (SOW)
Location: Huntsville VA CBOC, 500 Markaview Rd NW, Huntsville, AL 35805
Scope of Work: Midmark 626-005 exam chairs with scale
Product and Delivery: Delivery and Installation of Exam Chairs and Lights. Contractor shall ship and deliver three (3) new 626-005 exam chairs and exam lights to the Birmingham, VA Medical Center.
Coordination: Contractor will coordinate with designated VA stakeholders at least two (2) weeks prior to the anticipated delivery date. The purpose of this coordination is to confirm delivery dates and times and to ensure that delivery and installation do not interrupt patient care or operations at the Medical Center.
Installation: Contractor shall install the three (3) new 626-005 exam chairs and corresponding exam lights at the designated locations within the Birmingham VA Medical Center, in accordance with manufacturer specifications and VA requirements.
Debris & Old Equipment Removal: Upon completion of installation, Contractor is responsible for:
a. Removing all shipping debris from the installation areas and medical center property.
b. Removing all existing/old exam chairs that are being replaced.
c. Proper disposal of all removed material and debris in compliance with regulatory guidelines and VA policies.
5. Project Completion: The project will be considered complete upon confirmation by VA stakeholders that all 3 new exam chairs and exam lights are installed and fully operational. b. All shipping materials, debris, and old chairs have been removed from the facility.
Additional Notes:
Any work affecting patient care areas must be scheduled to minimize interruption to clinical operations.
Contractor shall provide a primary point of contact for project coordination and communication.
