BRIEF SCOPE OF WORK
(This is for information Only)
PURPOSE
The purpose of this requirement is to deliver two (2) Chiropractic Table to VA SLC Utah Health Care System, located at 500 Foothill Drive Salt Lake City, Utah 84148.
SCOPE
The Contractor shall deliver all items listed
All items must meet all salient characteristics defined
The Contractor shall furnish all personnel, supplies, and equipment required for delivery.
ITEM SPECIFICATIONS AND SALIENT CHARACTERISTICS
Detailed Item Description Example: Chiropractic Table
Quantity: Two (2) each
Technical Specifications/Salient Characteristics
Height will range between 21 ½ -29 (Optional 24.5 32 ) controlled by electric foot pedal
Length must not exceed 77
Width must be the standard 27
Must include Tilting Headpiece and Flexion Headpiece
Must have manual forward and straight drop headpiece
Will include control panel touchscreen
Lateral Flexion must move and lock up to 21°
Weight of table not to exceed 300lbs
Must support lifting capacity up to 450lbs
Armrests will have patient gripper bars
Must have air activated thoracic breakaway, thoracic drop with release, lumbar drop, pelvic drop, and lateral flexion with locking mechanism
Must have slide out ankle support with harnesses
Must be compatible with electric requirement of 110V 60Hz-7A-5A or 220V-50Hz-4A
Minimum one (1) year warranty on all parts
DELIVERY AND INSTALLATION
Delivery Timeframe: within 30 days of award unless specified differently within scope of contract.
Delivery Location: Contractor shall deliver all equipment to the VA SLC Utah Health Care System, located at 500 Foothill Drive Salt Lake City, Utah 84148.
Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufactures s name marked thereon. Deliveries shall be marked with the PO and contract number.
OPERATION AND MAINTENANCE MANUALS
Binders - Quantity (1) each for items identified
Digital Copies - Quantity (1) each for items identified
Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to On-Site Point of Contact upon completion of installation.
SECURITY REQUIREMENTS
The C&A requirements do not apply. A Security Accreditation Package is not required.
WARRANTY
The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery.
The warranties shall include all travel and shipping costs associated with any warranty repair.
