Are you now, or have you in the last 5 years been a subcontractor on federal contracts for Pest Control Supplies? If so, list agencies and prime contractors. Are you a manufacturer or authorized distributor? If manufacturer do you contract directly with the government, or do you only use utilized authorized suppliers? If supplier, which companies are you authorized to supply for? Can your organization/company support all the Veteran Medical Centers for pest control related products in all 50 US states and US territories? If not, please indicate states or US territories that you do not serve (e.g., Alaska, Hawaii, Puerto Rico). What EPA approved (if required) and/or pest management products do you provide for the healthcare environment? Are your products registered in all 50 US states?
