PERFORMANCE WORK STATEMENT FOR 72 OSS AIR TRAFFIC CONTROL TOWER WINDOW SHADE INSTALLATION 1. DESCRIPTION OF SERVICES: The contractor shall provide all labor, tools, parts, materials and transportation required to install window shades at the Air Traffic Control Tower, 72 OSS/OSAT, Tinker AFB, Oklahoma: --20 primary window shades (full replacement including custom cut cords with bracket, black tassels & cord lock pulleys) --20 secondary window shades (full replacement including custom cut cords with bracket, white tassels & cord lock pulleys) Window shades must be in compliance with FAA Order 6480.18 design specifications The contractor shall provide the government with a designated point of contact and phone numbers to enable continuous contact during regular work hours. This work includes, but is not limited to, the following: 1.1. On-site setup: Contractor shall contact Tinker Air Traffic Control Tower at 405-734-5232 for escort onto the airfield. 1.2. Installation: Contractor will remove old shades and deliver and install the new shades. Contractor will dispose of old shades. 1.3. Documentation: The contractor shall provide complete documentation, all manuals, and warranty information. 1.4. Cleanup: The contractor is responsible for cleaning up the area and disposing of old shades. 1.4.1. The Contractor shall keep the work areas clean and neat IAW industry standards, OSHA, fire and safety standards, and this SOW. 1.5. Inspection: The contractor shall inspect to ensure the new shades are undamaged during shipping and are operable after installation. 1.5.1 Services shall include inspection of product for shipping damage. If damage is found it shall be the responsibility of the contractor to notify the GPOC and make all arrangements for replacement or repair of the item(s) damaged. Damaged items are not to be installed unless otherwise directed by the Contracting Officer or GPOC. -- 1 of 3 -- 1.6. Notification: The Contractor shall notify the GPOC once the materials have been shipped and provide an estimated delivery date. The Contractor shall provide all material handling required to move the materials from the delivery truck to the site of the installation. 1.7. Site Visit: If a site visit is required the Contractor shall provide knowledgeable personnel that can perform all needed tasks to complete measurements and design suitable for quotation within the time provided by Contracting for the site visit. 2. WORK SITE: Contractor shall follow all base regulations while operating on Tinker Air Force Base. 3. HOURS OF OPERATION: The Air Traffic Control Tower is open 24 hours a day/7 days a week. However, the facility will only have escort personnel available from 7am to 11pm, so work should be scheduled during those hours. Scheduling should be mutually agreed upon by both parties at no additional cost to the government. If work hours are required outside the hours stated, the Contractor shall contact the Government POC for approval from the CO. Upon approval from the CO, the Government Subject Matter Expert (SME) or Government POC must be present with the Contractor. 4. GENERAL INFORMATION: 4.1. Physical security: Contractor shall be responsible for safeguarding tools and equipment. Should government property be provided for contractor use, contractor shall be responsible for safeguarding property and securing at the close of their work period. 4.2. Conservation of utilities: Contractor will be working during normal duty hours, therefore there are no special utility conservation requirements. 4.3. Environmental protection & Safety: The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational health and safety. 4.4. Security police/security requirements: The government will provide general on-base Security Police service. Security Police telephone extension is 911 for emergencies and 734- 3737 for non- emergencies. 4.4.1. The contractor shall provide the names of all personnel requiring entry authorization to Tinker AFB. The contractor and/or representatives must have a valid Driver’s license and car insurance verification. MANUFACTURER' S WARRANTY: The contractor shall provide a standard parts & labor warranty. Contractor shall not be responsible to replace parts that are damaged or broken as a result of negligent use by the customer or acts of mother-nature. Clint J.
