Statement of Work: Spokane VA Backflow Preventer Testing and Inspection 1. General Information The purpose of this contract is for the Contractor to inspect and test the backflow preventers and cross‑connection devices installed at the Spokane VA Medical Center. 1.1 Project Title Spokane VA Backflow Preventer Testing and Inspection. 1.2 Project Background Annual inspection and testing of backflow prevention devices are required to protect potable and non‑potable water systems through verification of proper operation and prevention of cross‑contamination. This contract supports those requirements for the Spokane VA Medical Center. 1.3 Scope of Work CLIN 1 – Inspection and Testing of 100 Devices:The Contractor shall provide all labor, travel, equipment, and materials necessary to perform annual inspection, testing, and certification of 100 backflow preventers and cross‑connection devices located at 4815 N. Assembly St, Spokane, WA 99205. The Contractor shall maintain an accurate inventory/log of all devices inspected and tested under this CLIN. CLIN 2 – Repairs for up to 20 Devices (If Required):The Contractor shall provide repairs for up to 20 backflow preventers, if deficiencies are identified during testing. Repairs may include component replacement, adjustments, and re‑testing as needed to achieve compliance standards. Repairs shall only be performed upon Government approval and documented as completed under CLIN 2. All inspections, certifications, and repairs shall ensure compliance with the latest standards including ABPA, TJC, ASSE, NFPA 25, NFPA 13, AWWA, OSHA, CFR requirements, and other applicable federal, state, and local regulations. 1.4 Performance Objectives The Contractor shall perform all inspections, testing, and repairs of facility backflow preventers in compliance with NFPA 13, 17, NFPA 25, ASME, ANSI, ASSE, ABPA, and all other applicable federal regulations concerning water safety and cross‑contamination prevention. 1.5 Deliverables Schedule Task 1 – Testing and Inspection Reports: Due within 10 calendar days after services are rendered. Task 2 – Company/Technician Certification: Submitted with all service tickets and inspection reports; company certification due prior to contract award. Task 3 – Backflow Preventer Survey: To occur annually with inspection and certification. 1.6 Place of Performance All work shall be performed at the Spokane VA Medical Center located at 4815 N. Assembly Street, Spokane, WA 99205. 1.7 COR Authority The COR may not change contract terms. All changes require Contracting Officer approval and a formal modification. 1.8 Hours of Work Normal hours are 6:30 a.m. – 4:00 p.m., Monday through Friday, excluding Federal holidays. Emergency service must be available 24/7. 2. General Requirements The Contractor shall furnish all labor, transportation, tools, materials, supervision, and management required to perform all services, including repairs and testing of up to 100 backflow preventers. 2.2 Testing and Certification Testing shall be performed per manufacturer recommendations. Deficiencies discovered during testing shall be corrected by the Contractor. 3. Travel The Contractor shall provide all transportation and travel necessary for contract performance. 4. Government Responsibilities The Government shall provide facility access and notifications of outages or required coordination. 5. Contractor Personnel Requirements All personnel must be certified and licensed to perform backflow preventer inspection, testing, and repair in the State of Washington. 6. Protection of Person and Property The Contractor must maintain safe working conditions and protect property from damage. 7. Security Requirements All Contractor personnel must check in with VA officials, follow all VA policies, and display visible identification at all times. 8. Hours of Operation Work must be coordinated with VA Mechanical/Plumbing Shop and performed during normal hours unless otherwise approved.
