STATEMENT OF WORK P/R: 2126406Y61J5596008 POC: Paint Shop Foreman Jann Blevins Cell: (410) 636-3256 Email: jann.a.blevins@uscg.mil Ship Superintendent James Johnston Cell: (410)-636-3095 Email: james.c.johnston@uscg.mil Location: USCG Yard, 2401 Hawkins Point Rd, Baltimore, MD 21226 GENERAL REQUIREMENTS: The work consists of providing equipment as required IAW the description below. Located at 2401 Hawkins Point Road Baltimore MD 21226, 270’ CGC Campbell. 1.1 Government intended use is for exterior surfaces of Coast Guard 270’ vessel while enclosed on ship lift. The work will include blasting and coatings operations of the entire U/W Hull surface of the vessel. Normal damage caused by the above work task will be at no cost to the Government. Rental will be at a firm fixed price regardless of the hours used. Any damage found outside normal wear and tears must be presented to Contracting Officer for approval prior to repair work being completed. Vendors are responsible for taking preventive measures to protect their equipment against wear and tear caused by conditions described in the statement of work. COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK: Upon acceptance the contractor shall provide the equipment for 7 consecutive months sometime between 07/6/26 and 02/28/27. Final period of performance will be determined by the Yard POC / Ship Superintendent. Description of Equipment: Description: JLG Articulating Boom Lift 800AJ 4WD or equal. Lifts must be manufactured within the 5 years of contract award date. An inspection report finding the equipment to be in good working order must be completed prior to delivery & forwarded to the Contracting Officer. (2) Articulating Boom Lift- Platform Height of 80ft, Platform Capacity of 500lbs, Wheelbase 9ft 6in, Platform Rotation of 160 Degrees, Ground Clearance of 15in. If at any time the equipment is deemed non-functioning, Contractor has 24 hours to either replace or have equipment up and functioning. Contractor must be available 7 days a week for repairs or replacement. Delivery: Equipment must be delivered to the USCG Yard 2401 Hawkins Point Road Baltimore Maryland 21226. Bldg. 80 location between 7:00am and 9:00am. Access to the Yard during the rental period is coordinated through the Yard POC. All personnel shall receive visitors’ passes and clearance from USCG Coast Guard Yard Security located at the Main Gate. List of personnel requiring access shall be provided to the Yard POC twenty-four (24) hours prior to requiring access. Personnel will be required to provide full name, picture I.D. (valid driver’s license preferred), and date of birth. For non-US citizens, a passport will be required. The following forms of IDS are authorized: CAC, Foreign Passport containing appropriate visas, TWIC, Green Cards, US Passport, or MOST state IDs. Pick Up: Request automatic pick up after 7 months unless optional weeks are authorized by Contracting Officer. Only the Contracting Officer may modify the contract. A contract modification must be executed prior to an extension occurring. Schedule Period: 7 consecutive months sometime between 07/6/26 & 02/28/27. Final period of performance will be determined by the Yard POC / Shop Superintendent. Damage to Government Property: The Contractor shall be held liable for any damage to government property caused by the Contractor. Any concerns must be brought to the attention of the POC. 8. Definitions and Acronyms: NA CONTRACT DEFINITIONS: CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime. CONTRACTING OFFICER. A person with authority to enter, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government. Hard Hats: The Contractor shall have the employee’s name, as it appears on identification card, designated on the front and the Company’s name on both sides. The lettering shall be standard commercial lettering (not hand printed), no less than ½ inch in height for the Company’s name and no less than 3 /8” in height for employee’s name. Lettering color is to be black. ORAL MODIFICATION: No oral statement of any person other than the contracting officer in any manner or degree modifies or otherwise affects the terms of this contract.
