Page 1 of 8 DISTRIBUTION: DoW COMMUNITY ONLY PERFORMANCE WORK STATEMENT (PWS) Philippines Base Life Support (BLS) (Amendment 1) 1. Background: US Forces consistently require Base Life Support (BLS) services in the Philippines. BLS is for the Initial Planning Conference for Balikatan (BK27). 2. General: This is a non-personal service(s) contract under which the personnel rendering the service(s) are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. 3. Description of Services/Introduction: This is a non-personal service(s) contract to provide Base Life Support to US government personnel. 4. Scope: Specific services for this Base Life Support contract include Generator Fuel, Room dividers, HAZMAT collection and MHE Capability,. 5. Deliverables: 5.1 Generator Fuel, 50-gallon drum of diesel Fuel, quantity: 2 drums. 5.2 Room dividers, mobile (wheeled) with dry-erase white board on one side and tackable cork board on the other side. Total length 238ft. Will need for 6 separate rooms in the following lengths: three x 23ft, 39ft, 52ft, 78ft. 5.3 HAZMAT collection (to include disposal) for US PACOM J76 generator maintenance. The type of HAZMAT will be generator oil and used coolant. Estimated amount of HAZMAT that needs disposal: 10 gallons oil and 10 gallons coolant. 5.4 MHE Capability: Forklift, 1 ton (capable of lifting 1000kg) with operator (for movement tents and generators). Must be able to travel on all-terrain. MHE Type: 1T Forklift. 6. Place of Performance: Camp Aguinaldo Generator Fuel, HAZMAT collection, Forklift - (in front of HADR warehouse) 14°36'17.3"N 121°04'03.2"E Room dividers - (CCC Annex) 14°36'21.2"N 121°04'05.0"E 7. Period of Performance: Generator Fuel: Delivery on Aug 17, 2026 Room dividers: 8/21/2026-8/29/2026 HAZMAT collection: 8/23/2026-8/23/2026 MHE Capability: 8/17/2026-8/23/2026 Operator will be required 0800-1800 on those dates. 8. Specific Tasks: 8.1 Generator Fuel: 50-gallon drum of diesel Fuel, quantity: 2 drums. Total length 238ft. Will need for 6 separate rooms in the following lengths: three x 23ft, 39ft, 52ft, 78ft. Contractor shall provide drop off and set up. 8.3 HAZMAT Collection and Disposal. The contractor shall provide temporary HAZMAT storage, disposal, and related services to isolate hazardous items in medical and other facilities. Typical medical disposal includes, but is not limited to, blood, bodily fluid, disease/sickness tests and contaminants, common medical chemicals and solutions, oils, infections waste, batteries, or combustible material. Includes, but is not limited to, HAZMAT trash bags, HAZMAT trash cans/storage and removal/disposal. STD: HAZMAT trash bags and cans shall have a biohazard symbol or label warnings. Bags shall be puncture resistant and compatible with the HAZMAT trash can. The Bags shall be in new condition, unused, and in packaging. The can shall have a step-on opener or another method to provide hands free operation. The can must be high-density polyethylene to resist rust and most chemicals. The cans must be seamless and leak proof. The bags and cans shall be OSHA compliant for exposure to blood borne pathogens. STD: Contractor shall remove/dispose the HAZMAT trash bags at the frequency specified at the PWS. HAZMAT will be disposed of by contractors in accordance with OSHA regulations and Philippine Law. Unit of Measure (UM): Estimated amount of HAZMAT that needs disposal: 10 gallons oil and 10 gallons coolant. The Contractor is required to dispose of all hazardous materials (HAZMAT) resulting from the US PACOM J76 generator maintenance, regardless of whether the final volume exceeds the initial estimate. Example HAZMAT Disposal 8.4 Material Handling Equipment with Operator (MHE). The contractor shall provide MHE and driver to facilitate the movement of cargo, equipment, and containers. Includes, but not limited to, MHE equipment and operator. The capability required and/or specified type of MHE will be 1 Ton forklift capable of moving containers and equipment weighing up to 1 ton (1000kg), MHE shall be able to maneuver through tight areas. Equipment and operators may be required to enter restricted areas (i.e. flight lines, etc.). When required, equipment operators shall be trained and qualified to drive on flight line and shall have approved flight line access. Operators shall also obtain all required placards and clearances necessary for entrance to perform with unlimited road access. All equipment and operators shall be licensed and authorized to operate at locations specified at the PWS. Equipment and operators may be required to enter restricted areas (i.e. flight lines, etc.). When required, equipment operators shall be trained and qualified to drive on flight line and shall have -- 2 of 8 -- Page 3 of 8 DISTRIBUTION: DoW COMMUNITY ONLY approved flight line access. Operators shall also obtain all required placards and clearances necessary for entrance to perform with unlimited road access. Operators. Equipment operators may be required to work a 12 hour period with possible night operations. Equipment operators shall be proficient in conversational Basic English and able to translate directions / instructions from the Government TPOC. Equipment operators will keep on their person at all times a valid license or equal documentation that may be required by the host nation as proof for legal operation of the vehicle/equipment in which they are assigned to drive/operate.
