Performance Work Statement Black Butte Dam & Lake Oxidation Pond Liner Replacement Project 31 March 2026 3 August 2026 1. Background: Black Butte Dam and Lake includes three recreation areas, each served by an independent, lagoon-based wastewater treatment system comprised of a septic tank (primary treatment) and oxidation pond (secondary treatment). The ponds vary in size and treatment capacity and have been in continuous operation since 1979. Wastewater generated at each recreation area receives primary treatment in septic tanks prior to discharge into the oxidation ponds. The ponds are engineered, man-made basins with large surface areas designed to provide secondary biological treatment through natural aerobic and anaerobic processes. In addition to treatment, the ponds provide effluent storage and seasonal volume reduction through evaporation. As a result of prolonged operational exposure, the existing pond liners have deteriorated and no longer provide reliable containment, creating the need for liner replacements. 3. Description: The required contractor services are to replace three (3) existing oxidation pond liners and restore them to fully operational sewage treatment systems. The work includes removal and replacement of the existing pond liners in the following recreation areas: • Buckhorn Recreation Area – approximately 12,500 square feet • Eagle Pass Recreation Area – approximately 8,500 7,000 square feet • Orland Buttes Recreation Area – approximately 8,000 square feet -- 1 of 13 -- The square footage values provided above represent the approximate liner surface areas and do not include additional material required for overlaps, anchor trenches, or other installation requirements necessary to ensure proper liner performance and system integrity. 3.1. General Requirements: The Contractor shall furnish all labor, materials, equipment, supervision, and services necessary to complete this project. All materials and equipment shall be suitable for their intended application and suitable for the environmental conditions in which they will be installed. 3.1.1. The Contractor shall be responsible for the complete installation of the pond liner systems and shall ensure the oxidation ponds are fully functional and watertight upon completion. The completed installation shall be free of leaks, defects, and installation deficiencies. 3.1.2. The Contractor shall inspect the project site to verify existing conditions, confirm quantities of work, and identify any conditions that may affect performance of the work. The Contractor shall establish and maintain any required perimeter controls, including but not limited to safety barriers and site access controls, as necessary to safely and properly execute the work. 3.2. Pre-Work Coordination and Submittals: The Contractor shall coordinate with the U.S. Army Corps of Engineers (USACE) Contracting Officer’s Representative (COR) to ensure a clear understanding of the project scope, requirements, and performance expectations. Prior to commencing on-site work, the Contractor shall conduct an in- person Pre-Work Meeting and submit a complete Pre-Work Submittal Package for Government review and approval. The Pre-Work Submittal Package shall include the Accident Prevention Plan (APP), Quality Control Plan (QCP), Work Plan, Liner Panel Placement Plan, and Waste Disposal Plan. These submittals shall be provided within 14 calendar days of contract award. The Government will review submittals within 5 calendar days. If revisions are required, the Contractor shall address all comments and resubmit within 5 calendar days of notification. The Contractor shall also complete all applicable requirements identified in Section 9.0, Antiterrorism and Operations Security, prior to mobilization. 3.2.1. Pre-Work Meeting: The prime Contractor shall coordinate with the COR to schedule an in-person Pre-Work Meeting to discuss all aspects of contract performance. At least 7 calendar days prior to the meeting, the Contractor shall submit a Certificate of Insurance and identify the onsite supervisor and alternate. The COR will establish the date, time, and location of the meeting. 3.2.1.1. Insurance Requirements - The Contractor shall procure and maintain insurance for all employees during the entire period of his/her performance under this contract. 3.2.1.1.1. Workmen’s Compensation and Employers’ Liability Insurance. 3.2.1.1.2. Workmen’s Compensation and Occupational Disease Coverage in -- 2 of 13 -- accordance with statutory limits. 3.2.1.1.3. Employers’ Liability coverage with a minimum limit of $100,000. 3.2.1.1.4. Comprehensive Automobile Liability Insurance: Bodily Injury coverage with minimum limits of $200,000 per person and $500,000 per occurrence. 3.2.1.1.5. Property Damage Coverage with a minimum limit of $20,000 per occurrence. 3.2.2. Accident Prevention Plan: The Contractor shall prepare and submit a site-specific APP prior to the start of on-site work. The APP shall comply with USACE EM 385- 1-1 and include all required elements outlined in Appendix A, “Minimum Basic Outline for Preparation of Accident Prevention Plan.” Activity Hazard Analyses (AHAs) shall be included as appendices to the APP.
