STATEMENT OF NEED
FOR
PROVISION AND DELIVERY OF MEDICAL DRY ICE TO THE 14th MEDICAL GROUP

1. DESCRIPTION OF SERVICES. This is a statement of need for the delivery of Dry Ice to the 14th
Medical Group, Columbus AFB MS (herein referred to as “Koritz Clinic”). The Contractor shall provide all
management, tools, supplies, equipment, parts, containers and labor necessary to deliver Dry Ice.
Quantity needed is 300 pounds of Dry Ice pellets delivered weekly to the Koritz Clinic. The contractor
may select Tuesday, Wednesday, or Thursday as the initial delivery day; however, delivery will occur on
the same day every week thereafter for the duration of the contract, unless there is a previously
coordinated exception, in accordance with paragraph 2.2 HOURS OF OPERATION.
2. GENERAL INFORMATION.
2.1. CONTAINERS/BINS. The Contractor shall ensure delivery bins provided are in good working
condition. The Contractor will not supply delivery bins that are compromised (i.e., cracking or
leaking) which can accelerate dry Ice evaporation and cause safety concerns. Delivery bins must
contain soft rubber wheels or be equipped with a method of transporting the bin, therefore,
avoiding the likelihood of damaging the flooring at transitions where linoleum flooring meets
ceramic tile and everywhere in between the loading dock and the laboratory.
2.2. HOURS OF OPERATION. Normal hours of coverage are Monday thru Friday, 0730-1630, excluding
Federal holidays and projected Family days. In the event of a planned closure, delivery will be
made the day prior to the closure date, then resume the normal delivery schedule the following
week and from then on. In the event of an unscheduled closure, the contractor will be notified as
soon as possible and before scheduled delivery.
2.3. SECURITY REQUIRMENTS. For Official Use Only (FOUO). The contractor shall comply with DODM
5400.07_AFMAN 33-302 “Freedom of Information Act Program” and AFI 33-332 “Air Force Privacy
and Civil Liberties Program”, requirements. The regulations set policy and procedures of records to
the public and for marking, handling, transmitting, and safeguarding FOUO material.
