Recurring Cleaning Schedule is required for approval within 10 business days of award. This exhibit does not need to be submitted for technical acceptability. Exhibit D – CDC Flooring Chemical Information The new Mondo Flex II floors at the CDC have specific guidelines for floor maintenance. These chemicals have been approved by Environmental Flight. -- 3 of 48 -- 4 PERFORMANCE WORK STATEMENT (PWS) FOR CUSTODIAL SERVICES 1. DESCRIPTION OF SERVICES. The Contractor shall provide non-personal services to include all management, supervision, personnel, labor, equipment, tools, paper products, and other items and services necessary to perform custodial services at Altus AFB. Services shall be performed in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. The Contractor shall accomplish all facility, restroom and periodic cleaning tasks to meet the requirements of this PWS and the Service Summary. 1.1. CLEANING FREQUENCIES. ALL cleaning frequencies are established in Appendix A and Air Force Common Levels of Service (AFCOLS). Facility cleaning levels and estimated square footages are established in Exhibit A, Estimated Square Footages and Level of Service. Facility drawings of areas cleaned will be provided to the Contractor upon contract award or as changes to square footage/buildings occur. All work performed by the Contractor shall be performed in accordance with (IAW) this PWS, all applicable laws, AFCOLS, other instructions or regulations and commercial practices. 1.1.1. The work performed under this contract will be done at approximately 89 facilities on Altus AFB. The Contractor’s on-site manager shall submit a preliminary cleaning schedule to the Contracting Officer Representative (COR) within 10 business days of contract award using Exhibit C. The schedule shall outline tasks to be performed by specific buildings. The on-site manager will work with the COR to come to a satisfactorily agreed upon cleaning schedule. Any temporary or requested permanent changes made to the schedule shall be submitted to the COR within 10 business days. The COR shall document the changes in an MFR if approved. This schedule will not include “as needed” services, which will be issued by a work order by the COR. 1.2. CLEANING, DISINFECTING AND SANITIZING SOLUTIONS. The Contractor is responsible to provide all cleaning, disinfection and sanitizing solutions. All chemicals must have HAZMAT approval (see paragraph 4.6.1) prior to bringing the chemicals on base. Chemicals MAY NOT be substituted without prior coordination and approval from the Civil Engineer Squadron (CES) Environmental Flight. 1.2.1. Chemicals must be stored in the original container or a container clearly labeled with the contents (i.e. – “Bleach Water”). All cleaning, disinfecting and sanitizing solutions shall be always kept out of the reach of children. 1.2.2. CLEANING SOLUTION. Use an approved commercial germicidal detergent to clean visibly soiled areas. Use of a scrub brush or other tool is authorized if it will not harm or damage the surface being cleaned. 1.2.3. DISINFECTING SOLUTION. Use an approved commercial disinfectant to prevent infection from non-food surfaces and equipment by destroying harmful bacteria and other microbes.
