Description

Statement of Work (SOW) Two-post Lift Replacement 1.0 Scope The seller shall provide two (2) new two post lifts with all associated components required and including old lift removal & lift installation in our shop. All work and materials shall meet the requirements outlined in this Statement of Work (SOW). 2.0 Applicable Parties  Purchaser: Air Force Global Strike Command (U.S. Government)  Receiving Unit: 341st Missile Wing, Logistics Readiness Squadron, Vehicle Management  Contractor: Referred to as “the seller” throughout this document 3.0 Requirements 3.1 Two Post Lifts The seller shall provide two (2) new (not refurbished, rebuilt, or used) two-post lift assembly, with a minimum lift capacity of 30,000 lbs, that meets the following requirements:  Must be ALI GOLD CERTIFIED  Must be able to be serviced/repaired by a vender on-site at Malmstrom AFB within 24-72 hours of notification. Absolute maximum allowable on-site response time shall not exceed five (5) business days. 3.2 Included Components The lift assembly shall include, at minimum:  4" sealed steel roller bearings  Structural 7/8” fork, lift mast channel  No Equalization Cables or Structural Overhead Beams  Three-stage telescopic, multi position, single lock, low profile arms  Automatic mechanical load holding locking device -- 1 of 2 --  Emergency Stop on control panel  Two 3hp motors  1in thick steel bolster plate  All required electrical wiring and hydraulic oil  Electronically operated control interface  Complete paint finish with markings All components must be new and fully functional upon delivery and installation. 4.0 Warranty The manufacturer shall provide a minimum 25-year warranty covering structural components, 2-year warranty on hydraulic components, and lifetime warranty on the carriage bearings. 5.0 Post-Installation Sustainment & Maintenance Plan With their proposal, the Contractor will include a sustainment plan describing how they will ensure that comprehensive maintenance, repair and annual certification services are available for the two-post lifts for the duration of the warranty and confirm if local/dispatched technicians are authorized to perform both ALI certifications and repair services. 6.0 Pricing The seller shall provide the total contract cost as one firm-fixed price covering all materials, components, and warranty requirements described in this SOW. 7.0 Inspection and Acceptance A representative from the 341st Missile Wing Logistics Readiness Squadron, Vehicle Management, will inspect the delivered lift assemblies prior to acceptance. Acceptance will occur only after the product is verified to meet all specifications in this SOW. 7.0 Delivery Delivery terms, schedule, and location shall be in accordance with the awarded contract and any applicable modifications. -- 2 of 2 --
