Description

Prospective offerors are encouraged to register their interest in the solicitation on the System for Award Management (SAM.gov) website so that they are notified of the issuance of the solicitation and any amendments. The contractor shall provide, at the contractor's facility, all necessary personnel, supervision, facilities, space, equipment and supplies to provide community-based outpatient care as listed in the Performance Work Statement. The purpose of the solicitation will be to obtain proposals from contractors who can provide community-based outpatient care for Veterans receiving care in and around the Franklin, LA area. The resulting contract will provide for five (5) one-year ordering periods, exercisable at the discretion of the Contracting Officer. Estimated period of performance is from 1 April 2027 to 30 March 2033. The Government will use a best value source selection method and anticipates making an award of firm-fixed price, indefinite delivery, and indefinite quantity contract for this requirement. PSC Code Q201 Medical – General Health, North American Industry Classification System (NAICS) Code is 621498 – All Outpatient Care Centers, and size standard is $25.5 Million. This solicitation will be issued as an SDVOSB set aside. All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in the SAM (System for Award Management) database (information on registration requirements can be obtained via the Internet at https://www.sam.gov ).
