PERFORMANCE WORK STATEMENT
FOR
MOLD ASSESSMENTS
AT
JOINT BASE SAN ANTONIO (JBSA)

1. DESCRIPTION OF SERVICES. The contractor shall provide non-personal services, that shall
include all personnel, equipment, tools, materials, supervision, and other items and services necessary to
provide an air quality and mold assessment for facilities located at Joint Base San Antonio (JBSA), to
include Camp Bullis, Canyon Lake Recreation Areas, Chapman Annex, Fort Sam Houston (FSH),
Lackland, Kelly Field, Randolph and Seguin Auxiliary Field. The purpose is to identify any potential
risks to personnel, and degradation of facilities or equipment. All work shall be performed in accordance
with industry and commercial practices, using commercial/industry standard products, equipment,
materials, supplies and tools. The contractor shall comply with applicable federal, regional, state, and
local laws as well as commercial standards to include obtaining the proper and required licenses,
certificates, insurance and permits.
1.1. CONTRACTOR QUALIFICATIONS. All Mold Technicians, Consultants or Companies shall be
Texas Mold certified according to the Texas Department of Licensing and Registration, Texas Title 16,
Part 4, Texas Administrative Code, Chapter 78, Mold Assessors and Remediators, (Effective September
1, 2018)
1.2. SERVICES REQUIRED. The contractor shall be responsible for providing all equipment, supplies,
materials, labor, etc. to accomplish all work necessary to meet the requirements of this PWS.
1.2.1. Risk Assessments. All assessment activities will be coordinated with JBSA prior to any
inspections. A mold assessment activity is an inspection, investigation, or survey of a dwelling or other
structure to provide the owner or occupant with information regarding the presence, identification, or
evaluation, collection or analysis of a mold sample and the development of a mold remediation protocol.
1.2.2. Contractor shall:
1.2.2.1. Conduct an indoor air quality assessment to include background air assessment, indoor air quality
assessment, which includes air temperature and relative humidity levels
1.2.2.2. Document and provide photos of any visible mold growth or signs of water intrusion.
1.2.2.3. Assess rooms, common areas etc., with thermal imaging technology.
1.2.2.4. Coordinate with JBSA Requester to determine Air Sampling requirements prior to an initial
assessment.
1.2.2.5. The person who conducts the mold assessment must be licensed in the State of Texas and in
accordance with the Texas Department of Licensing and Registration.
1.2.2.6. Perform all sampling in accordance with accepted methodologies and within regulatory
requirements as appropriate.
1.2.2.7. Provide a written report within 5 days of completion of all work. Report shall include
photographic documentation and sampling results and meet the requirements of the Mold Assessor
Administrative Rules.
1.2.2.8. Sampling results shall be from American Hygiene Association (AIHA) certified and appropriately
licensed laboratory.
When visual mold equals 25 contiguous square feet or more, the contractor shall prepare a Mold
Remediation Protocol following the mold procedures and all other activities required by the Mold
Assessors Administrative Rules, Title 16, Part 4, Texas Administrative Code, Chapter 78, Mold Assessors
and Remediators.

1.3. CLEARANCES.
1.3.1. Clearances will be conducted 24 hours after completion of a Mold Remediation Activity
1.3.2. Clearance shall consist of visual and Lift Tape Tests as required
1.3.3. Mold Protocol and Lab analysis reports shall be completed within 10 days.
1.4. INSURANCE. Contractor shall obtain commercial general liability insurance in the amount of not
less than $1 million per occurrence and maintain the coverage for the term of the assessor license.
1.5. WORK ORDER PROCEDURES. Indoor Air Quality Assessments will be requested in writing on
a call sheet/form that is submitted by the Contracting Officer Representative (COR) and will be
completed within the time frame mutually agreed upon between the COR and contractor. Indoor Air
Quality assessments will be based on the unit pricing established on the applicable CLIN.
2. QUALITY CONTROL AND ASSURANCE. The government shall inspect and evaluate the
contractor’s performance to ensure services are received in accordance with requirements set forth in
this contract. The COR shall inspect by validating actual work performance, physically checking an
attribute of the completed task, checking a management information report, investigating customer
complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine
whether or not performance meets the standards contained in this PWS. When a performance threshold
has not been met or contractor performance has not been accomplished, the contractor will re-
accomplish the work at no additional cost to the Government.
3. PERSONNEL.
3.1. PRIMARY REPRESENTATIVE. The Contractor shall provide a contract manager who will be
responsible for ensuring all work is provided and accomplished under the contract. The contract
manager (or an approved alternate representative) should be available to conduct overall management
coordination and furnish liaison with the Government. The contract manager will be the point of contact
with the Government and will have the authority to act or make decisions for the Contractor on all
matters pertaining to this contract.
3.2. The Contractor shall provide the name, mailing address, email address, and telephone number(s) for
the contract manager to the Government Program Manager within at least five (5) calendar days post
award. The Contractor shall verbally notify the CO of any changes as they occur and provide written
changes not later than five (5) calendar days prior to the effective date of the change in contractor
personnel under this contract.
