RFQ INFORMATION AND INSTRUCTIONS FOR PREPARATION OF QUOTATIONS A. RFQ INFORMATION This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only RFQ; quotes are being requested and a written RFQ will not be issued. This is an OPEN-MARKET Firm Fixed Price (FFP) RFQ. This RFQ is prepared in accordance with FAR Part 12, Acquisition of Commercial Products and Commercial Services, in conjunction with the policies and procedures for RFQ, evaluation, and award as prescribed in FAR Part 13, Simplified Acquisition Procedures. The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2026-01 effective 03/13/2026. A Defense Priorities and Allocations System (DPAS) rating is not applicable to this procurement. This is a TOTAL SMALL BUSINESS SET-ASIDE RFQ, and the North American Industry Classification System (NAICS) code is 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers. The Small Business Size Standard is 200 employees. A responsible quoter is one whose quotation conforms to the RFQ and will be most advantageous to the Government. INSTRUCTIONS FOR PREPARATION AND SUBMISSION OF QUOTATIONS (FAR 52.212-1 Addenda) All questions concerning this RFQ shall be submitted electronically to the Agency contact listed above no later than the due date and time for questions regarding this RFQ. Each prospective Quoter should attempt to state each question in that the Quoter would have no objection to the Government publishing that precise question (and its answer) in a formal amendment to the RFQ. Likewise, each prospective Quoter should attempt to word each question in such a way that the publication of that question would not divulge any information that the prospective Quoter considers to be proprietary or confidential. With the submission of its quote, the entity submitting a quote warrants that, to the best of the Offeror’s knowledge and belief, there are no relevant facts or circumstances which give rise to an organizational conflict of interest, as defined in FAR Subpart 9.5, or that the Offeror has disclosed all such relevant information.  In responding to this RFQ, the Contractor shall address an understanding of logistics, schedule, and any other miscellaneous issues of which the Government should be aware. The Contractor shall indicate if any technical assumptions have been made, conditions have been stipulated or exceptions have been taken with the Government Description of Requirement as written. If technical assumptions are not noted, it will be assumed that the vendor’s quote reflects no technical assumptions for the award and agrees to comply with all the terms and conditions set forth herein. By submission of its quote, the contractor accepts all terms and conditions, representations and certifications, and technical requirements. The Government intends to select ONE Contractor for the award of this requirement. The Government has a requirement for ISCU Drug Testing Supplies. Therefore, the Government plans to award one contract as a result from this RFQ. Quotations shall be submitted electronically as Adobe (pdf) files in an email whose size shall not exceed twenty Megabytes (10MB) no later than the due date and time [for Questions] cover page 1 of this RFQ. If multiple emails are required, they shall be identified in sequence (1 of 3, 2 of 3, etc.). Document(s) shall not contain any security restrictions (e.g. password protection) that prevent the Government from distributing and printing the document(s) and shall not include marketing materials or letters of support/recommendation. Quotations submitted shall be valid for a period of no less than ninety (90) days from the closing date of the RFQ. Offerors shall not be reimbursed for the costs of developing a quote for this RFQ. Quotations MUST include the following: Attachment A-1: Cover Letter, Attachment A-2: Solicitation Pricing Sheet, and Attachment A-3: Mandatory Response Question, in the format described in section B.1.2 and the Price Quote as described in section B.1.3; and Acknowledgment of all solicitation amendments, if any.
