Description

Please complete the SF1449: Blocks 17 (to include the CAGE code and the Unique Entity ID Number) and 30 A - C. Quotes received after the deadline will be considered untimely and will not be eligible for award. Questions are due by NLT 10:00 AM/CDT on 14 July 2026. All questions must be submitted in writing to patrick.a.thielemann.civ@health.mil and salameya.paulouskaya2.civ@health.mil. Electronic submissions with files larger than 20MB will need to be submitted in separate emails. All emails, regardless of the size, need to be received by the Government server, by the stated date and time. It is the offeror's responsibility to ensure the Government receives the entire quote. Factor 1. Technical Capability: The technical proposal shall be addressed in sufficient written detail for the Government to determine if the Offeror understands the technical aspects of the Government's requirement. Offeror shall demonstrate that it can provide required supplies that are compatible with Medi-Dose Information Labeling Technology (MILT) software via the submission of a written capability statement, product literature, or other materials. Factor 2. Price: Offeror shall complete the pricing for each of the contract line items (CLINS) in the attached Pricing Worksheet.
