United States Department of the Interior U.S. FISH AND WILDLIFE SERVICE 5275 LEESBURG PIKE, MS: NWRS FALLS CHURCH, VA 22041-3803 STATEMENT OF WORK DRAFT – ASBESTOS, LEAD BASED PAINT, AND HAZARDOUS MATERIALS BUILDING ASSESSMENT SURVEYS AT Upper Mississippi River NFWR-Winona District (UMR NFWR-Winona District) 1. SCOPE The U.S. Fish & Wildlife Service (USFWS) Upper Mississippi River NFWR-Winona District (UMR NFWR-Winona District) located in Winona County, MN is requesting a Request For Quote (RFQ) to perform all work necessary for and incidental to the inspection of structure(s) prior to building renovations, demolition and/or deconstruction to determine the presence of asbestos, lead, and hazardous materials until (INSERT TIME) central time on (Day, Date INSERT). Any Request for Proposals received after that time will be rejected for consideration. The Request for Quote will be awarded to the bidder who is qualified and has the lowest price. This Scope of Work is requested to provide sufficient sampling, testing and an assessment for Asbestos Containing Material (ACM), Lead Based Paint (LBP), and hazardous material prior to the demolition of building. Buildings listed as scheduled for demolition shall include destructive testing to determine presents of asbestos. Destructive testing shall be sealed to prevent damage from water until the building can be removed. Note: A limited previous asbestos survey was conducted previously and is provided to assist in conducting this inspection. The survey shall be performed in applicable safe working conditions as defined by State of Minnesota, Occupational Safety and Health Administration (OSHA) and the United States Environmental Protection Agency (EPA). The RFQ(s) are being solicited from qualified firms to perform site inspections of the building listed in the Attachment #1. The site inspection will entail inspecting both the interior and exterior of the building for asbestos, lead, and hazardous materials as needed, and supplying UMR NFWR-Winona District with a written report indicating the presence of asbestos, lead, and hazardous materials. The report will identify the type of material, condition, location, and accurate square and/or linear footage. Cost estimates for removing any confirmed asbestos containing materials, lead impacted areas, and hazardous material shall be provided for each type. Two (2) written reports (one (1) to the Station, one (1) to the Regional Environmental Compliance Coordinator (RECC)) and an electronically submitted report to the station and the RECC. -- 1 of 9 -- The building surveys shall be performed in accordance with Regulatory Agencies: 1) Environmental Protection Agency (EPA) 2) Occupational Safety and Health Agency (OSHA) 3) State of Minnesota a) Minnesota Department of Health (MDH) b) Minnesota Department of Occupational Safety and Health Administration OSHA (MNOSHA) The building surveys shall be performed in accordance with: a) Asbestos 1) NESHAP - 40 CFR Part 61, Subpart M – National Emission Standard for Asbestos 2) TSCA - 40 CFR Subpart D, Section 763 – Asbestos 3) 29 CFR 1910.1001: General Industry Occupational Safety and Health Standards 4) 29 CFR 1926.1101: Safety and Health Regulations for Construction Minnesota Rule, Parts 4620.3000-3724, Asbestos Abatement Standard 5) Minnesota Statutes, Section 326.70 to 326.81, Asbestos Abatement Act b) Lead 1) TSCA - 40 CFR Subpart D, Section 745 – Lead-Based Paint Hazard 2) 29 CFR 1910.1025: General Industry Occupational Safety and Health Standards 3) 29 CFR 1926.62: Safety and Health Regulations for Construction 4) Minn. Stat. §§ 144.9501-144.9512 – Lead Poisoning Prevention Act Before a firm will be awarded a contract, the firm must have supplied to the U.S. Fish and Wildlife Serviced the firm's resume with business references and the following information particularly relative to the activity being undertaken in this project: 1. Name and address of submitter (Contractor), plus name and telephone number of the individual to contact for further information. 2. Persons conducting inspections for asbestos must be certified by the MN Department of Health – Asbestos Program. 3. Persons conducting the inspection for lead must be licensed as Lead Assessor or Inspector by the MN Department of Health – Lead Program. 4. Name of lab being used to analyze asbestos samples and the National Voluntary Laboratory Accreditation Program (NVLAP) Asbestos Fiber Analysis Program Certification Number. 5. Name of lab being used to analyze lead samples and National Lead Laboratory Accreditation Program (NVPAP) Certification Number. 6. If an XRF Lead Analyzer is used to analyze lead paint samples, a) the company will provide XRF Machine Serial Number and documentation of calibration of unit. b) the company will registration of XRF Machine with State of Minnesota Registration. 7. All proposal shall include a Sample Report (Asbestos, Lead) with Individual Identification Number for Sample, Location of Sample, Description of Sampled Material, Sample Result, Estimated Square Footage or Linear Footage with Descriptions (i.e. 6 Inch Pipe Insulation, 6 Foot Pipe Insulation) of Material. 8. Any other information which will demonstrate abilities to perform there required services should also be included. -- 2 of 9 -- 9. Proof of Insurance. 2. PERIOD & PLACE OF PERFORMANCE Period of Performance: The Contractor shall have (Select one: 15, 30, 45, 90, 120) days to complete the survey.
