Department of Defense Defense Health Agency Performance Work Statement [Univeral Patient Home Exercise Program] [NHCP Physical Therapy] Statement of Work/Performance Work Statement NMRTC Camp Pendleton Universal Patient Home Exercise Program Software for Physical/Occupational Therapists and Therapy Assistants BACKGROUND: The Physical/Occupational Therapy (PT/OT) Departments of Naval Hospital Camp Pendleton (NHCP), and all associated branch clinics currently do not have a universal repository of home exercises programs (HEP) for providers to disseminate to patients. Providers have relied on searching for exercises on opensource websites, use their own personal subscriptions to HEP, draw pictures to illustrate HEP demonstrated to patients, or use patient’s personal devices to record them performing HEP. When patients transition from one provider to another, there is often inconsistency in the HEP carried over, and because there is not universal HEP, some exercises may be reviewed and performed differently than originally intended. The lack of consistency compromises patient care significantly, and some providers are disadvantaged if they do not have access to a universal HEP to ensure their care meets updated evidence-based practice. Moreover, the providers do not have a universal method of encouraging and tracking compliance of HEP for patients. Providers may rely on patient report, or if they have a personal subscription to HEP software, they can utilize this additional resource to facilitate patient compliance. The lack of a universal system to track HEP compliance also compromises patient care, especially if patients transition from clinic to clinic, provider to provider. SCOPE: The Contractor shall provide a universal HEP program or software that: A. Utilizes updated evidence-based PT/OT practice. B. Is updated regularly as a web-based system, and not a paper-based program. C. Provides a user interface for the provider that allows for expedient HEP selection during a patient encounter. D. Allows for HEP to be disseminated to the patient via printed copy, email, or SMS text. E. Offers additional software application or “app” for the patient to track engagement and compliance of HEP. Equipment to be installed: A. Annual Subscription to HEP program or software for up to 63 providers in the PT/OT departments in Naval Hospital Camp Pendleton and Branch Clinics Yuma and Port Hueneme. Universal Clinical Training for Physical/Occupational Therapists and Therapy Assistants BACKGROUND: The Physical/Occupational Therapy (PT/OT) Departments of Naval Hospital Camp Pendleton (NHCP) and all associated branch clinics do not have a universal clinical training program to ensure that all providers are providing care to patients with the most up to date evidence-based practices. The PT/OT providers have varying degrees of experience, have trained in different locations, and are credentialed in different states that have a wide range of requirement for continuing education. Additionally, the PT/OT departments across Camp Pendleton, Yuma, and Port Hueneme are comprised of 12 clinics physically separated, with staff providing patient care separated from each other. 63 providers in this department have a wide range of skills, and there is no universal clinical training program to ensure that all staff are providing the most up to date evidence-based practice. Patients may receive different care from one clinic to another, and from one provider to another. This lack of consistency in provider practice compromises overall outcomes for patients. Current CEU content provided to PT/OT providers from DHA do not provide PT/OT specific training to enhance provision of care. SCOPE: The Contractor shall provide a universal clinical training program that: A. Is web-based and updated with the most up to date evidence-based practices for PT/OT B. Provides PT/OT specific training that is applicable to the patient population at NHCP. C. Provides CEU credits to providers as needed. Equipment to be Installed: A. Annual Subscription to Universal Clinical Training Program for up to 63 providers in the PT/OT departments. OBJECTIVES/GOALS: To have the PT/OT departments across Camp Pendleton, Yuma, and Port Hueneme and all its providers equipped with a universal HEP program and Clinical Training Program to enhance and unify the standard of care, in order to enhance patient outcomes and medical/operational readiness of our warfighters. Tasks/Requirements: See Scope above. GOVERNMENT FURNISHED PROPERTY/TASKS: Government will provide the network access for installation or utilization of software. SECURITY REQUIREMENTS (IF ANY): None PLACE OF PERFORMANCE: Naval Hospital Camp Pendleton and Branch Clinics Yuma and Port Hueneme PERIOD OF PERFORMANCE: 1 Year annual contract, with option to renew for 2 additional years. BASE YEAR: 25 SEP 2026 – 24 DEC 2026 OPTION YEAR 1: 25 DEC 2026 – 24 DEC 2027 OPTION YEAR 2: 25 DEC 2027 – 24 DEC 2028
