Portland Area Indian Health Service for Youth Residential Treatment Center Services

PORTLAND AREA OFFICE PERFORMANCE WORK STATEMENT SCOPE OF WORK Provision of Services A. The overall goal of this contract is for a Portland, Oregon based youth residential treatment facility serving eligible youth from Federally recognized tribes in Oregon and other states. The facility will provide residential care 24 hours a day, 7 days a week, and 365 days a year to assist youth to recover from complex substance use disorders and related consequences of problematic use. The facility will provide culturally respectful and responsive care to youth, their families and communities as funding and staffing capacity permit. Other services may be provided, including educational programming, medical and emergency referrals as needed, and referrals for mental health services. Youth generally present with complex trauma, significantly impairing mental health disorders, psychosocial difficulties, behavioral problems, and other conditions. B. To ensure the availability of the services and procedures stated herein in the Portland, Oregon metropolitan area, the Contractor shall provide the services either: 1. On-site, pursuant to written policies and procedures that conform to the standards of care and practice applicable to each service and, where appropriate; or 2. Through a subcontractor(s) with whom the Contractor has a written agreement. The agreements shall identify, for example, services, fees, response time, performance standards, and other requirements. C. The Contractor shall provide residential drug and alcohol treatment and other treatments deemed appropriate treatment for youth from federally recognized tribes in Oregon and other states, with care and services offered 24 hours a day, 7 days week and 365 days a year. The contractor will establish age ranges of youth served based on state and other guidelines. Contractor will offer at least the minimum number of group and individual treatment, educational and process group care, case management and other services according to state and best practice clinical guidelines. The clinical programming is offered as per certifying and accreditation requirements. Community based offerings will also be offered as appropriate and as staffing and funding capacity allows. D. Contractor provided services shall include, but not be limited to, providing comprehensive, therapeutic substance use treatment to residential youth clients. Care delivery may include but is not limited to: group care, psychiatric referrals, medication management (on site or referred as needed), individual substance use disorder treatment, relapse prevention, suicide prevention activities, patient counseling and education, and other care. E. Contractor clinical staff shall maintain all applicable certifications and licensure. F. The Contractor shall utilize standards of care and practice regarding electronic data collection and documentation of beneficiary use, including current Oregon State law, and shall use a health information technology (HIT) system, e.g., the Resource and Patient Management System (RPMS) or Commercial Off the Shelf systems (COTS). -- 1 of 11 -- PORTLAND AREA OFFICE PERFORMANCE WORK STATEMENT G. The Contractor shall utilize a HIT software for collection and data entry of IHS requirements for the following information, if applicable: patient demographics, clinical data, behavioral health data, diabetes data, public health nursing data, immunization data, referrals, dental data, laboratory and radiology data and shall submit data to the IHS data center on a monthly basis. H. The Contractor shall provide proper and adequate clinical services in accordance with the Standards of Care established by recognized health care organizations including those of the Indian Health Service. All services shall be provided within accepted professional standards and be consistent with Commission on Accreditation of Rehabilitation Facilities (CARF) requirements. Services A. HEALTH CARE DELIVERY SYSTEM The purpose of this contract is to elevate the health status of the American Indian/Alaska Native (AI/AN) youth population residing in the Portland Area (OR, WA, ID). This is to be accomplished through the following specific efforts: B. RESIDENTIAL SUBSTANCE TREATMENT SERVICES The Contractor will provide an Adult residential substance abuse treatment program, a primary health care clinic, outpatient mental health services, outpatient substance abuse treatment, in addition to the Youth Residential Treatment Center. The Youth Residential Treatment Center serves youth ages 12-17 primarily in Portland, Oregon metropolitan area for eligible AI/AN from all over the nation, as well as referrals through Oregon Health Plan and DOJ for underserved youth. They facility to provide fully integrated treatment model reflecting traditional Native American healing approach of the Medicine Wheel -towards restoring a state of balance to the physical, emotional, mental and spiritual dimensions of the client’s life. The 24 bed unlocked residential facility serves both male and female youth with typical courses of treatment ranging from 90-180 days. American Indian and Alaska Native cultural practices are incorporated into all aspects of program activities. The client’s tribal cultural traditions are seen as an individual strength that can provide an important source of support for their recovery from alcohol and drug dependency. The client may choose to include a variety of traditional Native American cultural activities in their treatment plan such as sweat lodge ceremony, fire ceremony, traditional drum, singing, flute playing, painting, beading and leather craft. Best-practice methods of substance abuse counseling are incorporated with Native American cultural perspectives to deliver quality care that is culturally specific to the needs of American Indian and Alaska Native families.
