STATEMENT OF WORK
1. SCOPE
1.1. Mobile Elevated Work Platform (MEWP) rental to support required elevated work.
2. APLLICABLE DOCUMENTS
2.1. Issues of Publications - The following documents of the issue date in effect on the date
of request for proposal form a part of this specification to the extent applicable, or as
specified herein. Within the specification, they shall be referred to by their basic
designation only.

2.2. Order of precedence - In the event of conflict between the text of this specification and
that of the references cited above, the text of this specification shall take precedence.
Nothing in this specification, however, shall supersede applicable laws or regulations
unless a specific exemption has been obtained.
3. REQUIREMENTS
3.1. JLG model 600AJ-HC3
3.1.1. Minimum 60’ vertical reach articulating boom with jib.
3.1.2. Diesel powered.
3.1.3. Tires will be foam-filled, solid pneumatic, or equivalent.
3.1.4. Operating platform
3.1.4.1. Equipped with a minimum of 3 approved and labeled fall protection tie-off
points.
3.1.4.2. Large enough for a minimum of 3 people.
3.1.4.3. Minimum 600-lb unrestricted capacity.
3.1.4.4. Minimum 750-lb restricted capacity to full height and 25-feet horizontal
reach at any height throughout the platform travel arc.
3.1.4.5. Will be equipped with a swinging gate.
3.1.5. No older than 3 model years from the start of the contract
3.1.6. Equipped with Original Equipment Manufacturer lifting points that allow lifting by an
overhead crane using a standard 4-legged sling, without the use of special handling gear.

4. RESPONSIBILITITES
4.1. Government
4.1.1. Equipment Technical Evaluation: Code 986 Engineering
4.1.2. Receipt Inspection: Contracting Officer Representative (COR), Technical Point of
Contact (TPOC), Zone Manager (ZM), and/or C338 Team Lead (TL). If none of
these are PNSY licensed operators, a licensed operator assigned by PNSY will also
be present in accordance with section 5.3 of this SOW.
4.1.3. Schedule equipment delivery & returns: COR, TPOC, ZM & C338 TL
4.1.4. Schedule equipment service, and repair: COR, TPOC, ZM & C338 TL
4.2. Contractor
4.2.1. The equipment will be operating around-the-clock under a rigorous deployment
performance schedule. Rentals will include unlimited usage hours within the
contracted period of performance.
4.2.2. Within four (4) hours of initial notification of the need for repair, the Contractor
shall have a qualified and equipped technician on the job-site fully prepared to
repair the rental equipment. The defective unit(s) shall be repaired within twenty-
four (24) hours (one day) of initial notification or, if the original defective unit(s)
cannot be made operational, replaced within forty-eight (48) hours (two days) of
initial notification. If the equipment failure is not the fault of the Government, the
Contractor shall bear all costs associated with its repair and/or replacement.
4.2.3. The contractors and non- PNS Government Agencies (NPGAs) shall be
responsible to read and understand the Work Site Operational Safety Environment
requirements of OSHECM Chapter 250 and can see an electronic copy of OSHECM
Chapter 250 (latest revision) by using the following web site:

4.2.4. The Contractor shall provide and require their personnel to wear personal
protection equipment (PPE) (e.g., hearing protection, eye protection, gloves, steel-
toed shoes or boots, aprons, masks, face shields, reflective vests, safety harnesses
for fall protection in accordance with OSHA CFR 1926 (construction) and CFR
1915 (shipyard) standards.
4.2.5. The Contractor is required to follow all appropriate lockout/tag out requirements
as specified by OSHECM Chapter 250 and CFR 1915.89. Contractor shall conform
to all applicable safety and occupational health standards published in the Federal
Register or any other Occupational Safety and Health Administration (OSHA)
Public Laws and ensure employees observe safety, traffic control, and fire
prevention requirements in effect at the site.
4.2.6. Government intended use is for submarine work that may include high pressure
water blasting of outside hull. Government must be able to install a temporary
pressure washer mount in the basket and a hose guide along the boom of each rented
unit. (Modifications have been approved by JLG Industries Inc., (Case #

00556132). Before they are returned, the government will completely remove all
modifications and ensure all boom lifts are in the same condition as received. The
government will be responsible for any damage caused by the temporary
modifications. Rental will be on a firm fixed price regardless of the hours used.
4.3. Maintenance
4.3.1. Surveillance Checks
4.3.1.1. The Government will conduct daily fluid level checks; however, the
contractor shall be responsible for adding fluids to the equipment to ensure the
loss of fluid is not due to a more serious mechanical issue.
4.3.1.2. Except for daily fluid checks, the contractor shall be responsible for all
materials and labor for preventative maintenance and repairs not arising out of
abuse or negligence by the Government.
4.3.2. Preventative Maintenance
4.3.2.1. The Contractor shall coordinate with the COR, TPOC, ZM & C338 TL, in
writing, to schedule preventative maintenance.
4.3.2.2. The contractor shall maintain a stringent maintenance program in
accordance with Manufacturers’ recommendations and applicable ANSI/SAIA
A92 specifications.
4.3.2.3. Upon completion of regularly scheduled PMs, contractor will leave a copy
