PERFORMANCE WORK STATEMENT (PWS)
FOR: Urology Surgical Navigation System & Support

INTRODUCTION:
This effort includes tasks and activities to perform Urology Surgical Navigation System & Support
services at the 88th Medical Group (hereafter known as the MTF), Wright-Patterson Air Force Base,
Ohio.
DESCRIPTION OF SERVICES:
The contractor shall perform the tasks outlined in the following paragraphs to fulfill the requirements of
this Blanket Purchase Agreement (BPA):
- Provide UroNavigation Fusion System for MRI Biopsies; Ultrasound System for Da Vinci
Robotic Nephrectomies; and the Cyber TM Thulium Laser for Holmium Laser Enucleation of the
Prostate (HoLEPs). The equipment is designed in providing clear images of the prostate with
ultrasound-guided biopsy images in real time. As well as excellent visualization of lesions and the
placement of biopsy needles. The vendor services are performed at this facility at least 1-2 times per
month.
- The contractor shall check in with SYMPLR in Operating Room Surgical Logistics and must be
current with all the requirements prior providing patient care. The technicians providing the service
must wear green paper scrubs.
- The contractor must provide a technician with at least 12 months of experience and certifications
to operate the Uronavigation and Thulium Laser equipment.
- Technician provides intra-operative equipment monitoring services with the direct instruction of
the Urology provider during surgical procedures.
- The contractor shall deliver the UroNavigation and Laser equipment a day prior to the surgery
date or the day of surgery to ensure machine availability and to avoid procedure delays or
cancellations. The contractor shall set-up the equipment in the delegated Operating Room and ensure
the system is operationally

functional for the procedures the next day. Prior to its use, any equipment brought into the facility
must be cleared through the Medical Group’s Medical Equipment Repair Center (MERC) prior to its
use on patients. For after normal duty hours the MERC on-call person shall be contacted for this
clearance.
- Provide coverage on a scheduled basis, with normal operating hours occurring Monday thru
Friday, 7:00am - 4:30pm, excluding Federal holidays.
- FEDERAL HOLIDAYS. The recognized Federal Holidays and the days they are observed can
be found on the US Office of Personnel Management website. The Federal Holidays include: New
Years Day, Martin Luther King, Jr's Birthday, Presidents' Day, Memorial Day, Independence Day,
Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.
- Transport, maintain, repair and calibrate contractor equipment used in performing the required
services. Proof of maintenance shall be available and on file after each maintenance service.
MTF:
- MTF shall initiate authorized calls and provide the quantities placed on the calls.
- The Government shall monitor the contractor’s performance. Additionally, the contractor’s
performance is subject to scheduled and unscheduled review by the QAE (Quality Assurance
Evaluator).
SPECIFIC TAKS:
The contractor will maintain all licenses, certifications, insurance, and other requirements to
legally provide Urology Surgical Navigation System & Support services at Wright-Patterson AFB
(WPAFB) Ohio in accordance with the orders of WPAFB physicians. Such maintenance will be
accomplished at no cost the government. The contractor will provide UroNavigation and Laser
services only. The contractor will be paid a

designated set-up/break-down fee and a designated per hour rate for services provided at WPAFB.
The fee schedule will remain in effect if both the contractor and the government agree.
However, if a new fee schedule is presented, it will not increase in price more than 6% for an
additional year. The fee schedule may only be changed once per year and the change must be
approved by the Contracting Office before going into effect. Providers wishing to use UroNavigation
and Laser services will indicate the need on the electronic surgical request (e-buckslip) in the "Does
this procedure require a medical vendor, special equipment, special instruments, and/or special
supply not in inventory?" box. The authorized caller list will be used to schedule and/or cancel
services. In the event of case cancellation where said Urology Surgical Navigation System & Support
services where previously scheduled, the contractor will be notified a minimum of twenty four hours
before the scheduled procedure. Notification less than twenty four hours of case cancellation where
Urology Surgical Navigation System & Support services were previously scheduled will require the
government to pay the set up/break-down fee to the contractor. This fee will be paid once per day of
cancelled procedures; even if more than one procedure is cancelled for a given day. The contractor
will provide the Operating Room scheduling office and the Operating Room equipment custodian,
Wright Patterson Medical Center (WPMC), a minimum of two weeks’ notice of any dates of non-
availability to perform Urology Surgical Navigation System & Support services at WMPC.
INVOICING:
The contractor must accept payment by Government Purchase Card (GPC). Invoices must be
submitted at the end of each procedure or at the end of the day of more than one procedure is
performed on a given day.
LIMITATION OF LIABILITY:
In no event is the contractor responsible for any loss or damage arising out of the following, as
applicable: (a) the services, unless attributable to the contractor's negligence or willful misconduct (the
contractor cannot guarantee or warrant the effectiveness of the services in any way), (b) patient care or
the underlying condition(s) of the patient, or (c) the supplies and equipment provided by WPAFB.
