Attachment 1 – Bid Form (Price Schedule) Offerors shall provide firm-fixed monthly prices for Contract Line Item Numbers (CLINs) associated with standard janitorial services (e.g., CLIN 0001 and corresponding option year CLINs). Prices shall include all labor, supervision, management, equipment, tools, cleaning materials, consumable supplies, disinfectants, transportation, insurance, permits, licenses, disposal costs, and all other resources necessary to perform the services described in the Statement of Work (SOW). Standard Services (CLIN 0001 and corresponding option year CLINs) shall be paid on a firm-fixed monthly basis. CLIN 0002 and corresponding option year CLINs (e.g., 1002, 2002, etc.) are established for Additional Emergency and Unscheduled Janitorial Services, which are outside the scope of routine custodial services. These services include, but are not limited to, emergency cleaning, spill response, unplanned sanitation requirements, deep cleaning requests, and other unscheduled janitorial support as directed by the Government. This CLIN is inclusive of all labor, supervision, materials, supplies, equipment, and disposal as required to complete the work. This CLIN is structured as a Not-To-Exceed (NTE) amount and shall not exceed $7,500.00 per contract year. It shall be used strictly on an as-needed basis. The quantity for CLIN 0002 is for administrative purposes only. Multiple emergencies or unscheduled janitorial tasks may be performed under this CLIN, provided the total cumulative amount does not exceed the established NTE value. Work under CLIN 0002 shall be performed only upon prior written approval from the Contracting Officer or authorized Government representative. The Contractor shall submit a detailed, itemized cost estimate for each requirement. The Government will review and determine price reasonableness prior to authorization of work. Payment shall be made only for services performed and accepted by the Government.
