STATEMENT OF WORK FOR Backflow Preventer Repair and Retest BASE CAPE COD May 11, 2026 PART 1 - GENERAL SUMMARY The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to repair, replace, and retest (6) backflow preventer devices at USCG Base Cape Cod (BCC) on Joint Base Cape Cod, Buzzards Bay, MA. The Contractor shall repair and retest (1) Watts, model # LF009, Serial # 3503, type RPBP, 3” domestic backflow device at building 3154. The Contractor shall replace and retest (1) Ames, model # Colt 400, Serial # XI1403, type RPBP, 6” fire backflow device at building 3160. The Contractor shall replace and retest (1) Watts, model # 909, serial # 244915, type RPBP, 6” fire backflow device at building 3170. The Contractor shall repair and retest (1) Watts, model # 009M2QT, serial # 274898, type RPBP, 1” domestic backflow device at building 3426. The Contractor shall replace and retest (1) Watts, model # 009M2QTRP, serial # 304172, type RPBP, 2” domestic backflow device at building 5203. The Contractor shall repair and retest (1) FEBCO, model # 805, serial # B16935, type DCVA, 4” fire backflow device at building 5205. REFERENCES Reference Standards: The following current standards and publications of issues currently in effect form part of this specification to extent specified: U.S.C.G. Shore Asset Management System Job Plan 1972 U.S.C.G. Shore Asset Management System 1973 310 CMR 22.22: Cross Connections Distribution System Protection ADMINISTRATIVE REQUIREMENTS Coordinate the work of this section with the work of BCC. Schedule repair, replacement, and testing in coordination with the COR and Coast Guard mission requirements. Conduct preconstruction meeting with COR, Contractor, and BCC personnel. Prepare and submit schedule and submittals to the COR. SUBMITTALS The Contractor shall submit the following deliverables in accordance with the table below. Copies of each deliverable shall be provided to the Contracting Officer and COR. Electronic submissions are acceptable. Submittals Frequency Submittal Schedule Contact information for the designated on-site working supervisor With quote Certificate of Insurance With quote Base Access Request Form As revised Within 5 days of contract award Safety Data Sheet (SDS) and product label for chemicals and materials As needed Prior to use Closeout Submittal As completed Withing 7 calendar days after completing work Safety Plan As revised 15 calendar days after award Qualification Statement As needed With Quote Safety Plan: The Contractor shall develop and submit a written Safety Plan to the COR or designated representative for review and approval prior to commencing work and whenever changes are made thereafter. Informational Submittals: Backflow Preventor Device Repair & Testing schedule: 15 days before commencing work, submit written work plan. Include scope of work identifying BFP devices to be repaired, replaced & tested. Qualification Statements: Show membership status, project experience, and certifications for: BFP Device Testing Contractor. Supervisor – Valid Massachusetts Backflow Prevention Device Tester Certificate issued by the Department of Environmental Protection. Technician - Valid Massachusetts Backflow Prevention Device Tester Certificate issued by the Department of Environmental Protection. WORK HOURS Hours of Operation: The Government's regular working hours are 7:00 am to 3:30 pm, Monday through Friday. At the direction of the COR, work may be coordinated after hours to accommodate the Coast Guard’s mission and occupancy requirements. No work shall be performed on Federal holidays or Saturdays and Sundays unless authorized by the Contracting Officer. PERSONNEL REQUIREMENTS AND QUALIFICATIONS Backflow Prevention Device Testing Technician: Licensed and certified in Massachusetts. On-Site Working Supervisor: The Contractor shall provide a Massachusetts certified Backflow Prevention Device Tester responsible for project oversight to manage Contractor personnel at the work site. This individual shall remain on site while all services are being performed. This individual shall have full authority to act for the Contractor on all matters relating to the daily performance of the contract work at the worksite(s). If the on-site working supervisor is going to be absent for an extended period of time (more than 4 hours), the COR will be notified, and the Contractor will appoint a qualified substitute. Employees: The contractor shall staff trained and experienced personnel who exhibit capability to perform this requirement. All contractor employees shall be trained in and knowledgeable in backflow preventor device testing and maintenance. Appearance/Uniforms: Contractor employee personnel shall present a neat appearance and shall be easily recognized via contractor-furnished uniforms. All Contractor personnel shall wear nametags furnished by the Contractor containing, at a minimum, the employee's last name, the company name, and job titles. Contractor personnel shall wear their security ID card at all times when working in any of the areas. Security Requirements: To gain base access, the Contractor shall fill out a Base Access Form and provide a list of names, driver’s license numbers, citizenship, and birth date information for all personnel working under this specification and obtain a Coast Guard Security Identification (ID) card for each of them from the Base Security Office. All personnel listed will be subject to a background security check. Any personnel that do not meet or pass this security requirement will be subject to removal from BCC and the project.
