STATEMENT OF WORK
SIX RIVERS NATIONAL FOREST
MAD RIVER RANGER DISTRICT
ROUTE 1 ROAD REPAIR - V2.
1 Statement of Work
The intent for this contract is to secure services for asphalt pavement patching of Route 1 (06N01) which
has multiple potholes and ruts on the Mad River Ranger District of Six Rivers National Forest.
Patch and repair damaged areas of the travelway to facilitate traffic use. Asphalt pavement patching
consists of first cleaning and removing the existing surface of any loose material, dirt, or other deleterious
material by approved methods. The prepared hole shall be sprayed on all sides with an emulsified asphalt,
and holes filled with a cold bituminous mixture. The finished surface shall consist of an even ditribution
and compaction of the cold bituminous mixture at every patched or treated area.
The contractor shall carefully examine the existing conditions and information, shall become familiar
with the nature and location of the work, site constraints related to weather conditions and traffic use.
The contractor shall examine the condition of the asphalt surface, and the nature and extent of external
factors that will be encountered during the surface treatment operations and any existing site conditions
that may affect the project, prior to the beginning of work.
The contractor shall provide all personnel, equipment, supplies, facilites, transportation, tools, materials,
supervision, testing, nonpersonal services, and all incidental items necessary to perform the requirements
of this SOW and associated contract award.
All work shall be done in accordance with applicable Specifications and Industry Standards. Where one
or more of these conflict, the most stringent shall apply to the prosecution of work. The attachments to the
RFQ/Statement of Work are hereby made part of this solicitation and any resultant contract.
Contractor shall, without any additional expense to the Government, be responsible for obtaining any
necessary licenses and permits, and for compliance with any federal, state, and local laws, codes,
regulations, and ordinances applicable to and required for performance of the project work.
2 Project Location
The project area is on the Mad River Ranger District of Six Rivers National Forest.
Mad River: From Eureka take Highway 101 South to Highway 36 near Fortuna, California. Take
Highway 36 east past the town of Dinsmore, continue east to the Mad River Ranger District Office.The
estimated time of travel is approximately 1 hour and 30 minutes.
Route 1: From Highway 36 east, approximately 10 minutes from the Mad River Ranger District make a
left onto Route 1 (aka 06N01).
3 Contract Time
The Contractor shall complete all work, including final site clean-up, within 60 calendar days from the
Notice to Proceed. Contract period shall not include time the Contractor is unable to safely access the

work sits for reasons beyond the Contractor’s control including closure of access to the project area for
fires, floods, other weather-related events or for the Government’s convenience.
The projected start date for this project is anticipated to be August 2026.
4 Project Clean-up
Project site cleanup includes the removal and disposal of all project generated trash, debris, and residue
off National Forest Land in accordance with all applicable local, state, and federal regulations and
requirements. Additional requirements are described in Section 9 Special Contract Requirements.
5 Special Conditions Affecting The Work
A. Physical Conditions. Physical site conditions As Shown on the Drawings and in the
specifications are the result of site investigations and surveys performed:
i. At the time of original construction and,
ii. During subsequent routine inspections.
The Government makes no representations related to site conditions except as specifically noted
in the drawings or the specifications. No inferences shall be made by the offeror or contractor. A
topographic survey was not completed at the site; therefore, grading plans and wall layout are
approximate and will require field adjustment.
B. Work Hours. The approved work week is: five days (Monday-Friday), between 6:00 AM and
6:00 PM, Pacific Time. Any schedule for site work on federal holidays, Saturdays, Sundays, and
outside the times stated above shall be submitted to, and approved by, the Contract Officer prior
to such work being performed. The Contracting Officer shall be notified within 24 hours of any
emergency work needed to be performed outside the approved work week.
C. Highway Permits. Contractor is responsible for securing any overload and over-size/width limit
permits required for the transport of materials or equipment to the project site. The Contractor is
responsible for conducting a thorough reconnaissance of all routes leading to the site.
D. Toilets. There are no facilities at or near the project site. The Contractor shall provide all
necessary sanitation facilities as required by this contract, OSHA, and the State of California
regulations.
E. Camping. Camping is not permitted on Government land except as may be specifically allowed by
the Contracting Officer. Camping is permitted in established Forest Service campgrounds with the
permission of the concessioner. Contractor shall comply with all terms and conditions of any
camping permit issued to allow use of premises. Any non-compliance will result in suspension of
work until compliance is achieved.
F. Water. Limited quantities of non-potable water for the project construction can be drafted from
local streams. Use Best Management Practices and do not block public access. Suction
hoses/pipes shall have a 3/8” of less screen. Specific Requirements apply to drafting from live
stream. Information can be obtained from the Orleans Ranger District Office.
G. Security. Contractor is responsible for the Contractor’s equipment and materials owned or
