STATEMENT OF WORK
WHITERIVER SERVICE UNIT, HAZARDOUS WASTE MANAGEMENT
WHITERIVER, AZ
A. Introduction:
The Contractor shall perform the Biohazard & Hazardous Pharmaceutical waste retrieval and
disposals for the Indian Health Services, Whiteriver Service Unit. The project is a component of
the Department of the Health and Human Services. The initial contract will be one (1) year, with
option years to extend.
B. Background:
The services required are to provide waste pick-up and disposal services to the Whiteriver
Service Unit. Waste pick-up and disposal services are needed for Direct Patient Care in the clinic.
C. Statement of Work:
This service contract is for the Biohazard Waste Retrieval and Disposal for the Indian Health
Services, Whiteriver Service Unit. Procedures shall include, but not limited to, the following:
1) Shall provide all necessary supplies and equipment to provide the required services
2) Shall provide pickup, transport, and disposal services for all infectious waste including trace
chemotherapeutic waste.
3) Shall provide 4 times per year pickup, transport, and disposal services for all hazardous
pharmaceutical waste including Resource Conservation and Recovery Act (RCRA) and non-
RCRA waste.
4) RCRA Pharmaceutical waste (up to 150 pounds annually) disposal with high temperature
incineration and disposal in lined waste landfill.
5) Pharmacy Formulary characterized 2 times per year.
6) Provide Containers for RCRA (P-, U-, F- listed and chemotherapeutic agents) and non-RCRA
waste
7) Shall provide clean, rigid containers (biohazard barrels) to the Government for use in storing
infectious waste between pick-ups. “BIOHAZARD” and/or the international “biohazard”
symbol shall be visible on all the containers. Provide appropriate containers for medical
waste, trace chemotherapeutic waste, etc.
8) Shall ensure the transport vehicle has appropriate placards to carry biohazard materials. The
transport vehicle shall be fully enclosed with lockable doors and sealed to preclude leakage
of liquids from the enclosed areas. The transport vehicle should provide unrestricted access
to the infectious waste containers for purposes of loading and unloading.
9) Transporter: The driver will adhere to all posted speed limits in the Government compound
and use extreme caution when maneuvering and parking the transport vehicle during
pickups. All the labor to remove filled containers from secured storage area, loading the
vehicle and replacing with empty containers in secured are is the sole responsibility of the
transporter/driver and replacing with empty containers in secured area is the sole
responsibility of the transporter/driver. The transporter shall provide a receipt of the pickup
and the receipt will be signed by the Environmental Services director or a representative.

10) Updates: any and all updates pertaining to the Biohazard waste retrieval and disposals
service or any changes to the contractor’s policy and procedures shall be communicated to
the Contract Officer’s Representative (COR) and the Contractor with confirmation from both
parties of changes or updates.
11) Estimated quantities are twenty-two (22) waste containers that would be removed would
be approximately 44/45 gallons on a weekly basis from the WRSU Biohazard area.
12) The Biohazard Waste Retrieval and Disposal will be picked-up once every week between the
hours or 6am and 8am, excluding Federal Holidays. Scheduled service date is to be arranged
between the designated COR and the Contractor.
13) Contractor shall comply with applicable Environment Protection Agency (EPA), Occupational
Safety and Health Administration (OSHA); 1920.120 Resource Conversation and Recovery
Act conform to Federal, Tribal, State, and local regulatory guidance and ordinances in
regards to Biohazardous waste, disposal, and applications.
D. Additional Information
Invoicing will be to the Whiteriver Service Unit, Environmental Services Department.
Invoice Platform Processing (IPP) will be used.
