Unrestricted no set-aside Responses to this notice shall include the following: (a) company name  b) address (c) socio-economic status, (d) point of contact (e) phone, fax, and email (f) SAM UEI number (g) Cage Code  (h) Tax ID Number (i) and must  provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor and provide the commodities described below.   BRAND NAME OR EQUAL - NLY SELLICK EQUIPMENT LTD OBJECTIVE: Heavy Duty Forklift Background Northport VA Medical Center, Environmental Management Service, Grounds Department, is seeking to acquire a large 6,000 lbs. 4-wheel diesel engine operated forklift. The current forklift is beyond economical repair as it is aging and needs constant repairs. Having a new forklift with the capabilities below will make it possible for the Grounds Department to maximize the increasing workload and operations. The proposed forklift is a Sellick SLP 60K4e-4PS, 4WD, Powershift, 4-Speed or equivalent model. Description 6,000-pound, 4-wheel diesel combustion engine and hydraulically operated with pneumatic tires. Required Optional Features Enclosed Cab - 85 OAH ROPS/FOPS certified Cab Heater - 24,700 BTU Block Heater 110V-SLP Outside Mirrors 60 Hook Carriage CLIII 6K3R 60 Hook Pair (190838) Loadguard 60 x 48 6/8 HK/SH 3 Spool Valve Block Scope The contractor will have the requested products delivered to Northport VA Medical Center, 70 Middleville Road, Northport, NY 11768. Contractor is responsible for installation of all accessories, pick up, freight, and delivery from point of origin to delivery at the Northport Veteran Affairs Hospital. The machine must be able to be locally maintained by a factory certified repair facility. Standard Features (or equivalent): Rated capacity Range: 5,000 to 8,000 @ 24 load center General: One-Piece frame with 10 degree oscillating rear steer axle Engine: Kohler 2.5 L Tier 4 Diesel, turbocharged 4-cyclinder, 74 hp. Includes dual element air filter, fuel/water separator, cold weather starting aid, vertical exhaust and requires no exhaust aftertreatment. Transmission: JCB 4-Speed fully automatic powershift transmission utilizes electro-hydraulic directional control and ECU controlled gear shifting. Axles: Drive-JCB PD70 axle with limited slip differential. Steer-4WD JCB SD 55 outboard planetary axle with torque proportioning differential Brakes: Service-Enclosed oil-immersed multi-plate disc brakes. Parking Brake-Cable-operated, manually adjusted caliper with ergonomically designed handle. Electrical:12-volt, 95 A alternator, 875 CCA battery . Lighting: 2 front and 2 rear LED work lights. Safety: Safety LED lighting includes brake lights and amber warning light, safety decal package, horn, backup alarm, FOPS & ROPS certified, neutral safety start. Instruments: Full instrumentation includes warning indicator lights and gauges. Gauges include: fuel level, hour meter, coolant temp, system voltage. Indicator lights: park brake, transmission pressure, transmission temperature, preheat, air filter restriction, low voltage. Operator Compartment: Fully adjustable suspension seat with retractable seatbelt, tilt steering and sealed operator s manual storage. Tire Size & Type: Drive (pneumatic) 15 X 19.5-14 ply; Steer (pneumatic) 12 x 16.5-12 ply . Hydraulics: Hydraulic flow rate: 25 gpm @ 2200 rpm. Steering: Load sense power steering with hydraulic pump assistance. Warranty: One year/2000 hours. Delivery Delivery and acceptance are at Northport VA Medical Center, Building 36 Warehouse, 79 Middleville Rd, Northport, NY 11768. If Warehouse cannot take equipment due to its size, delivery directly to Building 16 Grounds is authorized. Equal Product: Offerors providing an or equal product(s) must adhere to the following terms. If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet or exceed are specified in the solicitation. To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must Meet the salient physical, functional, or performance characteristics specified in this statement of work. Clearly identify the item by- Brand name, if any and Make or model number Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer and Contracting Officer Representative. Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. Period of Performance Receipt within 180 days of award. Contracting Officer and Contracting Officer Representative can extend date if contractor provides evidence of prolonged backorder. The Government is not obligated to, nor will it pay for or reimburse any costs associated with responding to this Sources Sought request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition
