Statement of Work Certification and Inspection for Bio Hoods Technical Point of Contac: Robert Whitehorse, Jr., Equipment Specialist robert.witehorsejr@ihs.gov (505) 368-6140 1 BACKGROUND The Indian Health Service, Northern Navajo Medical Center (NNMC) in Shiprock, NM is requesting for four (4) bio hood (biological safety cabinet) recertifications and inspections. There are Three (3) bio hoods at NNMC and one 1 (one) at Dzilth-Na-O-Dith-Hle Health Center (DZ) facility. The bio hoods are required to be certified semiannual (twice-yearly) for proper operation. OBJECTIVES The objectives of the biohood certifications are to ensure the equipment provides a safe, sterile environment for the laboratory work involving infectious or hazard materials. The certification also protests the lab work, the product and the environment. The bio hood certifications are required to meet National Sanitation Foundation (NSF), the Joint Commission Accreditation for Healthcare Organization (JCAHO) and Occupational Safety and Health Administration (OSHA) regulations. PERIOD OF PERFORMANCE Services are performed twice a year within the timeframe below: Base Year: 11/24/2025 to 11/23/2026 Option Year 1: 11/24/2026 to 11/23/2027 Option Year 2: 11/24/2027 to 11/23/2028 Option Year 3: 11/24/2028 to 11/23/2029 Option Year 4: 11/24/2029 to 11/23/2030 TASK Services for the following Hood Fume Bacteriological: Item Make Model Serial Number Location Hood Fume Bacteriological Nuaire NU-540-300 179393032717 NNMC – GC128 Hood Fume Bacteriological Nuaire NU-540-600 179394032717 NNMC - GC134 Hood Fume Bacteriological Nuaire NU-301-436 185905051418 NNMC – LA141C Hood Fume Bacteriological Nuaire NU-540-400 186350060818 DZ Lab – B127 -- 1 of 3 -- Statement of Work Certification and Inspection for Bio Hoods Technical Point of Contac: Robert Whitehorse, Jr., Equipment Specialist robert.witehorsejr@ihs.gov (505) 368-6140 2 • The certification process must meet the critical test to ensure it meets the required standards America National Standard - NSF/ANSI 49 • Testing will confirm to Manufacturer’s Specifications, OSHA, NFPA 99-2005, ASSE 6030, • Federal Standard 209E, ISO 14644-1, USP 797 and U.S. • A certified bio hood technician is required to perform the services • Contractor is responsible for per-inspection prior to award • Contractor will notify the TPC at least two weeks in advance prior to conducting services • Contractor will conduct Certification in accordance to re-certification criteria: • Airflow testing (rate) • Air exchange - percentage on air count and inflow • Perform minor repairs necessary such as repairing small gaskets leaks or defects • Electrical safety test • Provide inflow and downflow velocity test in the cabinet to measure the velocity of air flow • Immediately notify TPC if certification fails and replace damaged and perform re- certification after repairs are completed. • Contractor shall update parts replacement, operator instructions or any procedural change. • Contractor shall provide a comprehensive service report that indicates the test result and maintained as a permanent equipment history file. This report will include description of work performed, man hours parts replacement numbers or nomenclature • Affix a certification sticker on each bio hood with clear certification date and expiration date • Telephone numbers for service calls and contract administration shall be provided to the Contracting Officer Representative (COR) and Technical Point of Contact (TPC). The COR and TPC will monitor the contract by review of the information provided with the bids, evaluation of actual work performed, response, timeliness, checking-in, service reports, general compliance with contract specifications and technical requirements. • The Contractor must see the COR/POC to get their signature on the service report and to be a legible photo copy of the services report; this constitutes “acceptance” by the COR/POC and a required for payment. -- 2 of 3 -- Statement of Work Certification and Inspection for Bio Hoods Technical Point of Contac: Robert Whitehorse, Jr., Equipment Specialist robert.witehorsejr@ihs.gov (505) 368-6140 3 GOVERNMENT-FURNISHED (PROPERTY) No government furnish supplies, equipment, etc. will be provided. The Contractor shall be responsible for safeguarding all government equipment, information and property provided to Contractor personnel while performing the services described herein. CONTRACT-FURNISHED (PROPERTY) The Contractor/Field Engineer or Technician, upon arrival at the facility, shall have all tools, test equipment, parts and manpower required for completion of the certification, inspection and repair services. SECURITY REQUIREMENTS Contractor shall check in with Facility Maintenance Staff and will escorted to security to obtain a Contractor’s visitor badge. All Contractors personnel are required to ware identification badges during the entire time they on in the hospital. Smoking is not permitted within or around the Healthcare facilities. Work shall be performed Monday through Friday from 8:00 am to 4:30 pm only.
