Statement of Work: Repair vent walls / structure, motors, and mounts damaged by wind storm.
Safety: The contractor will submit a Site Safety Specific Plan/Accident Prevention Plan in industry
recognizable format to the Contracting Officer within 7 days of contract award. The Plan shall include phone
numbers and maps to the local Dr. Office and nearest Hospital.
All employees of the general contractor or subcontractors shall have the 10- hour OSHA certified
Construction Safety course and/or other relevant competency training. Supervisors shall have completed the
30-hour OSHA training.
Base Bid: Provide all equipment, labor, and materials necessary (including dumpster/container if
applicable)
• Provide all quotes as turnkey completed work.
• Work shall be accomplished in accordance with all local, state, and federal codes and regulations.
• Work shall be accomplished by certified technicians.
• Repair/Replace vent wall panels ensuring integrity and operation.
• Repair/Replace vent wall motors, drive assembly, mounting brackets, mounting posts, etc to ensure
vent wall operation.
• Provide new mounting hardware necessary.
• Verify proper operation of vent walls with existing Wadsworth Controllers.
Contractor Responsibility:
1. Contractor will verify all dimensions and site conditions prior to beginning work.
2. Contractor will perform the work as listed in the above outlined Base Bid.
3. Contractor shall keep work area clean daily and take measures to control debris while work is being
performed.
4. Contractor is required to manage the project and consult with the Government prior to beginning all
work phases and to work in a manner that minimizes the impact of the work on Government
operations, research, and functions.
5. All work must be performed in such a manner as to not adversely affect daily operations of the
facility, its workers, and the onsite facility operations.
6. Contractor must always have a Site Supervisor on the job while working on this project.
7. Contractor will provide their own restroom facility and dumpster.
8. Contractor will remove all demolished material and debris from the worksite and dispose offsite.
9. The Contractor shall confine all operations on Government premises to areas authorized or approved
by the Contracting Specialist/Officer or Representative (CO/COR).
10. Employees of general contractor and subcontractors shall comply with ARS security management
program, obtain permission from the CO/COR, be identified by project and employer by wearing
contractor issued identification badges (if required) at all times on ARS property, and restricted from
unauthorized access.
11. The Contractor shall hold and save the Government, its officers, and agents, free and harmless from
liability of any nature occasioned by the Contractor's performance.

Material / Submittals:
The Contractor shall supply all materials necessary to successfully complete the project and deliver a
functioning system as required. Submittals on all material incorporated into the work, including shop
drawings and specifications, used on this project shall be submitted to the Contract Specialist/Officer (CO)
and Contracting Officer Representative (COR) as noted in this document. Electronic submittals are
acceptable otherwise three written submittals are required.
Demolition Procedure:
1. Contractor will verify all dimensions and site conditions prior to beginning work.
2. Contractor will schedule the work between working hours as listed below.
3. Contractor is required to provide labor, specified material, equipment, and other tools required to
perform the job fully and completely. No Government equipment will be available for use by the
contractor.

7. The contractor shall always be responsible for protecting their work and ensuring the safety of their
crew and ARS employees. Upon completion of the project, the contractor shall remove from the
premises all accumulated materials and rubbish caused by the project. Premises shall be left in a
clean, orderly, and acceptable condition.
8. All items demolished shall become property of the Contractor and be removed by Contractor from
ARS property.
9. Contractor shall follow all ARS EMS disposal criteria. Recyclable materials shall be recycled, and the
savings shall be included in the contractor’s quote, therefore passed on to the government.
10. Contractor shall be responsible for providing disposal records where applicable.
Damage to Property:
Any damage to the Government buildings, utilities, etc. caused as the result of the contractor’s activities shall
be repaired or replaced to the satisfaction of the Government.
Inspection and Acceptance:
A government representative shall inspect and approve the work performed on this project. The Contractor
shall schedule inspection a minimum of 10 days in advance, in writing, to the Contracting Officer.
Project Schedule:
The completion of this project shall not exceed 30 days from date of Notice to Proceed. A request for time
extension must be submitted to the Contracting Specialist/Officer in writing. Each request must include
specific and detailed reasons in support of the schedule alteration.
ARS reserves the right to close down or shut down the project site and order the Contractor’s employees off
