Department of Health and Human Services Indian Health Service Wastewater System Statement of Work Construction of a New On-Site Wastewater System for SR# 589 R Keller 1.01 PURPOSE OF THE PROJECT A. The Oklahoma City Area Office (OCA), Indian Health Services has a requirement for furnishing and installing a wastewater system as shown on the drawings. 1.02 SITE LOCATION A. 3376 Thrasher Road. White Cloud, Brown County, Kansas 66094 1.03 CONSTRUCTION SCHEDULE A. Contact the Engineering Technician three business days before the start of any work. B. Failure to perform work on the date scheduled shall require rescheduling the work 24- hours in advance. C. Leave all work exposed until it is inspected and approved. Polyvinyl Chloride (PVC) pipe, fittings and cleanouts shall be PVC schedule 40 as specified in the bid schedule. 2. Solvent weld, bell ended joints, pipe and fittings, conforming to ASTM D 3212 3. Four (4) inch nominal diameter and longest pipe length available. B. Septic tank: 1. 1,000-gallon septic tank. 2. Access holes in tank lid shall be cast-in-place with a minimum diameter of 20 inches. 3. Inlet and outlet sanitary Tees, 4-inch schedule SDR35 PVC. C. Effluent Filter and Assembly: 1. Filter rated for 800gpd flow rate. 2. 68 linear feet of 1/16 inch filter area. 3. Gasket seal to prevent effluent bypass. 4. NSF/ANSI certified and approved. 5. Baffle to fit 4-inch Schedule 40 pipe. 6. ¾ inch PVC lifting handle for filter that extends to within 2 inches of bottom of access lid. 7. Polylok, PL-68 Effluent filter or approved equal. D. Adapter ring, access risers and covers. 1. Concrete tank adapter ring shall be of solid design and prevent movement of soil around the riser. Arc3009 Polylok, 24 inch, or approved equal. 2. Riser shall be a solid, double-walled polyethylene pipe. 24-inch N-12 by ADS Pipe, or approved equal. 3. Cover shall completely cover riser so to prevent gases from leaking out and be of solid construction. Covers to be Polylok 3008-West, 24-inch HDPE flat septic tank cover or approved equal. 4. Joint sealant applied between the riser and adapter and the adapter and tank. Sealant to be a bitumous plastic joint sealant. Ram-Nek RN 103, or approved equal. E. Solvent weld, bell end 4-inch nominal inside diameter, PVC sewer pipe and fittings conforming to ASTM D2729. 2. Synthetic cover for trenches, a. TYPAR geotextile Style 3151 or approved equal. 3. Gravel. a. Gravel shall be clean and vary in size from ½ - 2-1/2 inches with no more than 10% by weight passing through a ½ inch screen. F. Chambers: Q4 Plus Standard Chamber or approved equal for the minimum requirement for an absorption area of 1125sqft. 1. Construct chamber sections of high – density Poly-Tuff Polyethylene. 2. Meet the minimum requirement of AASHTO-H-10 for wheel load protection. 3. Standard chamber sections shall be a minimum 34 inches in width, 53 inches in length and 12 inches in height. 4. End Caps of chamber sections shall be of the same material as the chamber. 5. Open End Caps: Provide an opening to accommodate 4-inch PVC Schedule 40 pipe. 6. Closed End Caps shall have no opening. 7. End caps shall have opening to accommodate for 4-inch PVC Schedule 40 pipe for inspections ports. G. Distribution / Drop Boxes: 1. Construct distribution/drop boxes and lids of concrete or injection molded HDPE. 2. Honeycombed and or cracked concrete boxes and lids are not allowed. 3. Inlet invert shall be 2 inches (minimum) higher than outlet inverts. 4. A 45o or 90o elbow shall be installed to direct water flow way from outlet port. 5. Box inlet shall accommodate for a 4” PVC Schedule 40 pipe and the outlet shall accommodate for 4” PVC Schedule 40 pipe. H. Turf and Grasses (Fertilizer, Seed and Mulch): 1. Fertilizer: 20-10-10 mixture of 20% nitrogen, 10% Phosphorous, and 10% Pot Ash. 2. Seed Mixture: Blend of Kentucky 31, Fescue, and annual rye grass. 3. Mulch: Prairie (native) hay, not more than 2 years old and free of noxious weeds. 1.08 LEVEL OF EFFORT A. General. 1. Construct the sewer system in accordance with the IHS Project Oversight’s drawings/design and authorization permits. 2. Variations from the drawing/design or authorization permit are not permitted. 3. All work required to bring the sewer system into compliance with the drawing/design and authorization permit shall be at the Contractor’s expense. B. Tank Installation: 1.
