Attachment A: Performance Work Statement Title: Laboratory Equipment Maintenance and Repair Services FDA Moffett Campus, 6502 S. Archer Rd, Bedford Park, IL 60501 1. BACKGROUND The Food and Drug Administration (FDA), Office of Laboratory Operations and Applied Science (OLOAS), Office of Applied Microbiology and Technology (OAMT), Division of Food Processing Science and Technology (DFPST) requires centralized maintenance and repair services for fifty (50) pieces of laboratory equipment located at the FDA Moffett Campus. A centralized approach ensures all equipment remains on a unified maintenance schedule, reduces administrative coordination burden, and maintains operational readiness in support of ongoing research activities. 2. SCOPE OF WORK The Contractor shall provide total assurance maintenance services for fifty (50) pieces of laboratory equipment as identified in Attachment B — List of Covered Equipment, located at the FDA Moffett Campus, 6502 S. Archer Rd, Bedford Park, IL 60501. Services shall include preventative maintenance and remedial repair services in accordance with Original Equipment Manufacturer (OEM) specifications. Preventative maintenance shall be performed in accordance with OEM specifications, and in no case less than one (1) visit per year per instrument. Unless otherwise noted, the maintenance program shall cover the cost of on-site repair service, including emergency repair, parts, labor, and travel, in addition to preventative maintenance visits per OEM specifications. Services shall be performed during normal business hours of 8:00 AM–5:00 PM, Monday through Friday, unless otherwise specified. 3. OBJECTIVES Maintain the performance of fifty (50) pieces of laboratory equipment as identified in Attachment B — List of Covered Equipment for excellent operable condition. Reduce the downtime of the equipment through preventative maintenance performed in accordance with OEM specifications, and in no case less than one (1) visit per year per instrument, and remedial repair services during the contract period using parts manufactured and certified by the manufacturer and service by manufacturer-certified engineers. Allow authorized end users — the individual FDA, OAMT scientists who use the equipment — to utilize their preferred service providers for most service events, to include OEM technicians, to perform the actual repair and preventative maintenance service on covered equipment. The cost of all covered service events shall be covered by the Contractor throughout the contract term. 4. SPECIFIC TASKS 4.1 Service Coordination and Dispatch The Contractor shall maintain a call center to receive maintenance and repair requests, staffed between the hours of 7:30 AM–8:00 PM, with phone support available outside of normal business hours when necessary. Equipment end users shall be provided with a toll-free number to request service or maintenance to be dispatched. Dispatch requests shall also be available via an online request form. The Contractor shall maintain a secure website available 24x7 for authorized representatives to request service and generate reports pertaining to the service agreement. Program transactions, equipment maintenance activity, and vendor information shall be available via the Contractor's secure website. 4.2 Preventative Maintenance The Contractor shall perform preventative maintenance on all covered equipment. Preventative maintenance shall be performed in accordance with OEM specifications, and in no case less than one (1) visit per year per instrument. Preventative maintenance shall be performed during normal business hours of 8:00 AM–5:00 PM, Monday through Friday. 4.3 Repair Services The Contractor shall perform or arrange all covered service events and shall bear all costs of covered service, including parts, labor, and travel, regardless of whether the service is performed by Contractor personnel, the OEM, or a qualified third-party service provider. The Contractor shall ensure that all service providers used are qualified to service the applicable equipment, perform quality workmanship, and deliver service within the response times specified in this PWS. 4.4 Software Updates Software updates are limited to those included by the OEM at no additional cost during corrective or preventative maintenance service visits. These updates typically address errors and/or reliability issues with an existing software license. Elective enhancements, upgrades, or other billable software maintenance shall not be covered under the agreement. 5. DELIVERABLES Preventative maintenance shall be performed in accordance with OEM specifications, and in no case less than one (1) visit per year per instrument. The Contractor shall perform remedial repair services as needed during the contract period. The Contractor shall provide service reports clearly identifying the work performed, in contractor format, submitted by email to the COR and designated POC after each service event. The Contractor shall provide a Monthly Service Summary Report identifying all equipment serviced during the reporting period, maintenance activities performed, and downtime and resolution status. All reports shall be submitted electronically to the COR and designated Points of Contact (POCs). 6. INSPECTION AND ACCEPTANCE Inspection and acceptance shall be performed at the place of performance (destination) by the Contracting Officer's Representative (COR). The Contractor shall notify the COR upon completion of each preventative maintenance visit and repair service event and shall provide written service reports as specified in Section 5. 7. CONTRACTOR PERSONNEL REQUIREMENTS Contractor personnel performing maintenance and repair services shall be OEM-certified or qualified technicians for the applicable equipment.
