Statement of Work (SOW) Medical Waste Service Contract 1. Title of Project: FPSU Medical Waste Service Contract 2. Background: The Fort Peck Service Unit, Verne E Gibbs Health Center located in Poplar, MT and the Chief Redstone Health Center located in Wolf Point, MT provides a wide range of medical service to members of the Assiniboine and Sioux Tribe. As a federally owned health care facilities accredited by The Joint Commission, there are federal and state requirements for properly disposing of regulated medical waste generated by the Emergency and Dental programs. 3. Scope: The contractor shall be responsible for removal and disposal of all regulated medical waste generated by each Clinic during the contract period of performance. The contractor shall also provide the customer adequate containers/bins/totes (historically a 28-gallon bin measuring 18” x 18” x 18” x 21”) and red bags for use in collecting Regulated Medical waste by our staff. Historically the Chief Redstone Health Center has had 12 28-gallon bins and 16 28- gallons bins at the Verne E Gibbs Health Center onsite. 4. Applicable Documents : Must meet all safety and CMMS accreditation standards. 5. Tasks: The typical collections are completed on two-week intervals and the quantities of bins collected at each location varies. Each bin will be removed and replaced at time of pick-up. The facility manager or his /her designee will visually verify all quantities collected and sign pick-up manifest. Contractor shall use methods that meet all state and federal regulations including but not limited to Montana Infectious Waste Management Act (MCA 75-10-1001 to 75-10-1007), Administrative Rules of Montana (ARM 17.50.1501 to 17.50.1508) and OSHA. 6. Deliverable and Delivery Schedule: The contractor shall two-week intervals and the quantities of bins collected at each location varies. Each bin will be removed and replaced at time of pick-up. The facility manager or his /her designee will visually verify all quantities collected and sign pick-up manifest. 7. Government-Furnished Equipment and Information: The government will provide secure storage locations for the medical waste bins for each facility. The government will store medical waste inside. Access to the storage shed will be given at each pickup service. -- 1 of 2 -- 8.
