STATEMENT OF WORK (SOW) Fire-Rated Door Repairs and Replacements Project: Housekeeping Entrance Fire Door Repairs and Replacements Location: Hopi Health Care Center 1. PURPOSE The purpose of this Statement of Work (SOW) is to obtain a qualified contractor to furnish all labor, supervision, materials, equipment, tools, transportation, and incidental services necessary to repair, replace, inspect, and certify fire-rated door assemblies at the Hopi Health Care Center. All work shall restore the designated fire doors to full operational condition and compliance with applicable life safety requirements. 2. PERIOD OF PERFORMANCE The Contractor shall complete all work within thirty (30) calendar days after receipt of the contract award. Work shall be performed during normal business hours: • Monday through Friday • 8:00 a.m. – 5:00 p.m. • Excluding Federal holidays Any work performed outside normal business hours shall require prior approval from the Government. 3. SCOPE OF WORK The Contractor shall provide all labor, materials, equipment, supervision, transportation, permits (if required), and disposal necessary to complete the following work. 3.1 Fire Door Replacements Replace two (2) failed fire-rated housekeeping entrance doors. Replacement work shall include all materials and hardware necessary for complete installation, including but not limited to: -- 1 of 6 -- • Fire-rated wood door slabs (45-minute rated) • Automatic flush bolt • Door replacement kit • Required clearance corrections • Door alignment and adjustments • Removal and disposal of existing doors • Functional testing following installation The replacement doors shall be installed in accordance with the manufacturer's recommendations and NFPA 80 requirements. 3.2 Fire Door Repairs Repair twenty (20) existing fire-rated doors. Repairs may include, but are not limited to: • Door adjustments • Clearance corrections • Installation of gap correction kits • Gravity coordinator replacement • Strike replacement • Strike shims • Smoke seal replacement • Replacement of defective fire door hardware • Hardware adjustments and operational testing The Contractor shall provide all replacement hardware identified during the inspection and listed in the attached schedule of required repair components. 3.3 Inspection and Testing Upon completion of repairs and replacements, the Contractor shall: -- 2 of 6 -- • Perform a complete operational inspection of all repaired and replaced fire doors. • Verify proper operation of all hardware. • Verify proper door clearances. • Ensure each fire-rated door assembly complies with NFPA 80 requirements. • Ensure work satisfies applicable Joint Commission standards. • Correct any deficiencies discovered during final inspection prior to project acceptance. 4. MATERIALS The Contractor shall furnish all materials necessary to complete this project. Replacement hardware shall be equivalent to the products identified in the attached hardware schedule unless otherwise approved by the Contracting Officer or Contracting Officer's Representative (COR). No substitutions shall be made without prior Government approval. 5. CONTRACTOR RESPONSIBILITIES The Contractor shall: • Furnish all labor, supervision, equipment, tools, and materials. • Employ qualified personnel experienced in fire-rated door installation and repair. • Perform work in a professional, workmanlike manner. • Protect Government property from damage. • Repair or replace any Government property damaged by Contractor personnel at no additional cost. • Maintain a clean and safe work area throughout the project. • Remove and dispose of all construction debris off Government property. • Coordinate work activities to minimize disruption to hospital operations. -- 3 of 6 -- 6. CODES AND STANDARDS All work shall comply with applicable editions of: • NFPA 80 – Standard for Fire Doors and Other Opening Protectives • OSHA Safety Requirements • Applicable Federal, State, and Local Codes • Hospital safety requirements • Joint Commission Life Safety Standards • Manufacturer installation requirements 7. QUALITY CONTROL The Contractor shall perform quality control inspections throughout the project to ensure: • Proper installation • Correct operation of all hardware • Compliance with fire door requirements • Completion of all identified repairs prior to Government acceptance 8. SAFETY The Contractor shall: • Provide all required Personal Protective Equipment (PPE). • Maintain a safe work environment. • Comply with OSHA regulations. • Prevent interference with hospital staff, patients, visitors, and emergency services. • Maintain safe pedestrian and vehicle access throughout construction. 9. CLEANUP At the completion of work, the Contractor shall: -- 4 of 6 -- • Remove all debris, packaging, and discarded materials. • Clean the work area. • Dispose of all waste at an approved off-site disposal facility. • Leave the work area in a clean, safe, and operational condition. 10. GOVERNMENT ACCEPTANCE Government acceptance will occur after: • Completion of all repairs and replacements. • Successful final inspection. • Verification that all fire doors operate correctly. • Confirmation that all work complies with NFPA 80 and Joint Commission requirements. 11.
