ATTACHMENT 02 Performance Work Statement Industrial Hygiene Technical Support Services Solicitation #W912JM-21-R-0015 21 pages 25 June 2026 Georgia Army National Guard USPFO for Georgia Regional Industrial Hygiene Southeast Office PERFORMANCE WORK STATEMENT (PWS) FOR Industrial Hygiene Technical Support Services 1.0 General: 1.1 Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary, except as specified herein as Government Furnished, to perform preform technical support for Industrial Hygiene (IH) Services, as defined in this Performance Work Statement (PWS). Background: This is a new award for same or similar services that have successfully been acquired by the Georgia National Guard in the past. This is a new acquisition for services to be performed during an established period of performance lasting no longer than one base period of one year, and 1 option period lasting one year each in duration. 1.3 Period of Performance (PoP): Commencement of work shall begin within 10 days after receipt of written Receipt of Award (ROA). The commencement of work shall begin within 10 calendar days after written receipt of award and last until the established period of performance end date of 30-September-2024, Period of performance extensions beyond this established end date will only be granted via fully executed supplemental agreement signed and issued by the Contracting Officer. Start Date End Date Base 1 September 2026 30 August 2027 Option Period 1 1 September 2027 30 August 2028 General Information: Normal hours of operation are Monday through Friday from 8:30 am to 5:00 pm Georgia time (no daylight savings). Except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Performance shall be at: Industrial Hygiene Region Southeast Office 510 Plaza Drive, Suite 1530 College Park, GA, 30349 The contractor shall always maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential. Teleworking is authorized. Telework: The Government will permit the contractor to telework in support of this requirement. In furtherance of Continuity of Operations Planning (COOP), a telework program may be enacted to ensure that the Government's mission-critical operations stay operational during times of national emergency or incidents of national significance. Telework shall be at no additional cost to the Government. Unscheduled gate closures by the Security Police may occur at any time causing all personnel entering or exiting a closed installation to experience a delay. This cannot be predicted or prevented. Contractors are not compensated for unexpected closures or delays. Vehicles operated by contractor personnel are subject to search pursuant to applicable regulations. Any moving violation of any applicable motor vehicle regulation may result in the termination of the contractor employee’s installation driving privileges. The contractor’s employees shall become familiar with and obey the regulations of the installation, including fire, traffic, safety, and security regulations while on the installation. Contractor employees should only enter restricted areas when required to do so and only upon prior approval. All contractor employees shall always carry proper identification with them and shall be subject to such checks as may be deemed necessary. The contractor shall ensure compliance with all regulations and orders of the installation, which may affect performance. The Government reserves the right to direct the removal of an employee from Government property or revoke access to Government systems for misconduct, security reasons, or any overt evidence of communicable disease. Removal of contractor employees for reasons stated above does not relieve the Contractor from responsibility for total performance of this contract. Recognized Holidays: The following are recognized United States (US) holidays. The contractor shall not perform services on these days: New Year’s Day: January 1st Martin Luther King, Jr.’s Birthday President’s Day Memorial Day Juneteenth: June 19th Independence Day: July 4th Labor Day Columbus Day Veteran’s Day: November 11th Thanksgiving Day Christmas Day: December 25th Quality Control (QC): The contractor shall develop and maintain an effective QC Plan (QCP) to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s QCP is the means by which it assures itself that its work complies with the requirements of the contract. As a minimum, the contractor shall develop QC procedures that address the areas identified in: Technical Exhibit 1, Performance Requirements Summary (PRS). A final QCP shall be submitted to the Contracting Officer Representative (COR) NLT 10 days after contract award. After acceptance of the QCP, the contractor shall obtain the Contracting Officer’s (KO’s) acceptance in writing of any proposed changes to its QCP. Quality Assurance (QA): The Government will evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government will do to ensure that the contractor has performed in accordance with the performance standards.
