1 PERFORMANCE WORK STATEMENT For PREVENTIVE MAINTENANCE, INSPECTIONS, SERVICE AND REPAIR OF MAN LIFTS AT JOINT BASE MCGUIRE-DIX-LAKEHURST, LAKEHURST, NEW JERSEY Joint Base McGuire-Dix-Lakehurst REVISION 1 13 MARCH 2026 -- 1 of 8 -- 1 PERFORMANCE WORK STATEMENT TO PROVIDE PREVENTIVE MAINTENANCE, INSPECTIONS, SERVICE AND REPAIR OF MAN LIFTS AT JOINT BASE MCGUIRE-DIX-LAKEHURST, LAKEHURST, NEW JERSEY 1.0 DESCRIPTION OF SERVICES. The contractor shall provide all labor, transportation, supervision, tools, materials, equipment, and management efforts necessary to provide preventive maintenance, inspections, service and repair of man lifts at Joint Base McGuire-Dix- Lakehurst (JB MDL), Lakehurst, New Jersey. All technicians must possess a valid manlift Certification to Repair and Operate all Manlifts. The level of services shall be in accordance with the terms and conditions indicated herein. During normal working hours (0800 – 1600) the contractor shall sign in at the 87th CES/CEOES, Lakehurst Building 5. 1.1 PROVIDE PREVENTIVE MAINTENANCE, INSPECTIONS, AND SERVICE CALLS OF MAN LIFTS. The contractor shall perform all preventive maintenance, inspections, and service of the man lifts listed in Appendix A, in accordance with manufacturer’s specifications, American National Standard Boom-Supported Elevating Work Platforms (ANSI/SIA), Occupational Safety and Health (OSHA), and Appendix B, Inspection Checklist. The contractor shall perform inspections, overhauls, and service of all systems and accessories listed under this contract, including Field Service that provides, but is not limited to, tire wear or damage, brake wear, filters (oil and air) and coolant changes, maintaining proper levels of coolants, batteries, oils, lubricants, hydraulic fluids. All inspections and maintenance shall be scheduled and coordinated with the Contracting Officer’s Representative (COR). An inspection and maintenance schedule shall be submitted for review to the COR 15 days after contract award. 1.2 REPAIRS. The contractor shall repair all mechanical / electrical / control / safety deficiencies as a result of the preventative maintenance / inspections on man lifts listed on this contract by a person (s) qualified as a mechanic on the specific make and model of the Aerial platform. The contractor shall perform these repairs at the contractor’s expense if the material cost is less than $100.00 per unit. The contractor shall absorb the labor expenditure. The contractor shall ensure all repair work commences within two (2) days of COR approval. The contractor shall ensure that all repair work is completed within five (5) days of its commencement; however, the contractor is encouraged to keep equipment downtime to a minimum. The contractor shall provide a written report not later than two (2) workdays to include but not limited to: mechanical / electrical / control / safety deficiencies found, actual parts replaced, parts cost, and man-hours per repair. A copy of the repair report shall be provided to the COR (Contracting Officer Representative). 1.2.1 MAJOR REPAIRS. If the repair work exceeds $100.00 in material, the contractor shall provide a written repair cost estimate to the COR and CO within two (2) business days of finding the deficiency requesting authorization to proceed. The written repair cost estimate shall include, at a minimum, the following: the estimated cost of material, the specified time needed to complete repairs, and the projected time the equipment will be out of service. The contractor shall be financially compensated for material and labor costs only if CO authorization is granted. -- 2 of 8 -- 2 The maximum financial threshold for major repairs is $5,000.00. At no time shall work in excess of $100.00 be completed without CO approval. The contractor shall ensure all major repair work commences within three (3) days of CO approval. The contractor shall ensure that all repair work is completed within seven (7) days of its commencement; however, the contractor is encouraged to keep equipment downtime to a minimum. The contractor shall provide a written repair completion report within two (2) days of job completion to include but not limited to: mechanical/electrical deficiencies found, actual parts replaced, parts cost, and man-hours per repair. A copy of the repair report shall be provided to the COR and CO. 1.3 SERVICE CALLS. In the event that a man lift fails or malfunctions, the contractor shall respond to service calls Monday through Friday 0700 AM – 0400 PM upon notification by the CO or the COR. The allowed maximum quantity of effort on a service call shall be 4 hours labor and shall not exceed $100.00 for materials. For any major repairs, the contractor shall secure the unit and use the procedures in paragraphs 1.2 and 1.2.1. The contractor shall conduct preventative maintenance, calibration and repair of the man lift in accordance with Paragraphs 1.1 and 1.2, if required. Due to the urgency of mission requirements, the contractor shall ensure that all repair work is completed within five (5) days unless a major repair is discovered resulting in additional repair time as denoted in Paragraph 1.2.1, Major Repairs. 2.0 SERVICE DELIVERY SUMMARY. The following are the performance thresholds for each service and represent the limits of “acceptable work”; any figures exceeding these thresholds shall be considered “unacceptable work”. Unacceptable work performed by the contractor must be corrected by the contractor within the required time frames set forth in the Performance Work Statement. Re-performance is the preferred method of correcting “unacceptable work”.
