Statement of Work For Illumina NextSeq 1000 Sequencing System Contracting Organization The Contracting activity for this action is the Defense Health Agency Contracting Activity (DHACA), 820 Chandler Street, Fort Detrick, MD 21702-5014. Customer Armed Forces Medical Examiner System DNA Operations (AFMES-DNA OPS), located at 115 Purple Heart Drive Dover Air Force Base in Delaware 1.0 Scope of Work The Government requires the awardee to deliver one (1) Illumina NextSeq 1000 Sequencing System. This procurement shall include the instrument, all necessary accessories for full operation, the onboard instrument control software (Linux CentOS), a three-year Silver Support Plan, and the specific XLEAP-SBS™ reagent and consumable kits to begin after the initial one- year manufacturer's warranty, comprehensive installation, and initial training. 2.0 Delivery Requirements The awardee shall be responsible for handling all shipping, handling, and insurance for the ground shipment of one (1) NextSeq 1000 Sequencing System and all associated accessories to the following address: AFMES-DNA OPS, 115 Purple Heart Drive, Dover Air Force Base, DE 19902 3.0 Installation and Qualification The vendor shall provide a manufacturer-certified engineer to perform the complete installation of the NextSeq 1000 Sequencing System and all its peripherals, including the integrated computer, and accessories. The vendor will perform a complete Installation Qualification/Operational Qualification (IQ/ OQ) in accordance with the manufacturer's specifications. This is to ensure the instrument is installed correctly and is functioning within all operational parameters as defined in the product's technical documentation. 4.0 Training and Support The training shall cover, at a minimum: Routine operation of the NextSeq 1000 Sequencing System. Full use of the instrument's software. All user-level maintenance procedures. The application specialist will provide support for the integration of the NextSeq 1000 Sequencing System with the existing AFMES Laboratory Information Management System (LIMS), ensuring that .txt format worklists can be properly imported and that all data reports can be correctly exported to the LIMS.
