Checklist for Offerors: Required Documents Offerors must submit the following documents with their proposal. Failure to provide any required document may result in disqualification. A. Administrative & Compliance Documents ☐ Signed Standard Form (SF) 1449 ☐ Completed FAR 52.212-3 Offeror Representations and Certifications ☐ Completed B.2 Schedule of Services and Price (all CLINs) ☐ Acknowledgment of all Solicitation Amendments (if any) ☐ Past performance information (recent/relevant contracts, references) ☐ Statement of agreement with all terms, conditions, and provisions (if not using SF 1449) B. Required Submissions (per SOW Section A.2) ☐ Letter describing: Make, model, year, and ID number of each vehicle to be used Location and telephone number of dispatch establishment Metering device or method for mileage determination ☐ Copy of current annual safety inspection (State of Missouri) for each vehicle ☐ Completed General Inspection Checklist for each vehicle (certified mechanic, Missouri-licensed facility) ☐ Certificate of Insurance (general liability, auto liability, workers’ compensation) with 30-day notice clause ☐ Proof of compliance with all Federal, State, and City codes (licenses, permits, etc.) C. Personnel & Training Documentation ☐ List of all drivers and attendants (names, roles) ☐ Copies of valid operator/chauffeur’s licenses for all drivers ☐ American Red Cross First Aid certificates (current, renewable every 3 years) ☐ American Red Cross Adult CPR certificates (current, renewed annually) ☐ Proof of oxygen dispensing training for all drivers/attendants ☐ Documentation of annual safety training (loading/unloading, patient handling) ☐ Documentation of infection control and bloodborne pathogens training (OSHA 29 CFR 1910.1030) ☐ Evidence of negative Mantoux (TB) test for all staff (within 14 days prior to service, annual re-testing) ☐ Hepatitis B vaccination records or declination forms for all staff ☐ Records of criminal background checks and health certificates for all staff ☐ Proof of pre-work orientation attendance (VA-provided) ☐ Annual competency certification/training records for all employees ☐ VA Cyber Security Awareness and General Privacy training certificates (if IT/sensitive info access required) D. Business & Legal Compliance ☐ Certification that no employees or subcontractors are listed on the OIG Exclusion List (www.hhs.gov/oig) ☐ Registration with Contractor Performance System (CPS) for contracts over $100,000 (https://cpscontractor.nih.gov) ☐ Business Associate Agreement (BAA) for HIPAA compliance (signed) ☐ Written exposure-control plan for infection control (OSHA compliance) ☐ Documentation of written policies for patient privacy/confidentiality E. Vehicle & Equipment Documentation ☐ Documentation of vehicle equipment (restraints, belts, fire extinguisher, first aid kit, oxygen tank, flashlights, sanitary blankets, climate control, communication devices, warning lights, audible signals) ☐ Daily maintenance/safety checklists (template/sample) ☐ Preventive maintenance records (template/sample) ☐ Proof of smoke-free vehicle policy F. Security & Access ☐ List of personnel requiring VA facility or IT access (with required personal info for background checks) ☐ Completed Special Agreement Check (SAC) Worksheet for each employee needing access ☐ Proof of completed background investigations (or in process) ☐ Proof of PIV card or Security Badge issuance (if applicable) Optional/Recommended for Clarity ☐ Table or flowchart explaining base rate vs. mileage rate scenarios ☐ Table or flowchart for no-load charge scenarios ☐ Table summarizing waiting time approval and documentation process Note: All documentation must be current, complete, and submitted as specified in the solicitation.
